This is a question that regards a patient who had a bigger right eye after n upper and lower eyelid blepharoplasty and was wondering if it is due to the surgery and whether it will get better. This is a question that I answered for someone who had a procedure by another physician. This persons right eye was larger than the left. This can happen with lower eyelid blepharoplasty when this procedure entails taking skin and fat. The skin resection and the scar contracture is what can cause the lower eyelid to retract downwards revealing more eye in that particular eye. This can take a while to resolve on its own. Sometimes, steroid injections can help.
In the long run, it may require other procedures to improve the retraction in the skin of the lower eyelid when the scarring occurs after lower eyelid blepharoplasty. This is one reason why I don’t prefer thenullas a procedure and believe in restoring volume to the lower eyelid and cheek region. I also prefer fat injections for this area for a longer lasting effect and more natural looking results. Sometimes this takes up to a year for the tightness to get better and for things to look better. Patience can be a virtue during this transition after a lower eyelid blepharoplasty.

Arcus Marginalis release is around the same price as null or a little more around $2k-5k. It depends on what region the surgeon is and the surgeons reputation. There are many ways to price fat grafting some use amount per milliliter or by region but for the lower eyelid and cheek area is around 3-k for fat grafting in the lower eyelid area.
Arcus mariginalis release and fat repositioning for the lower eyelid is a way to improve the appearance of baggy eyelids and fat bags in the lower eyelid. This does improve the uneven levels in the lower eyelid but it doesn’t correct a fundamental thing–a loss of volume. Also, the complications are high with this procedure. You can have problems with your eyelid retracting, the fat often retracts back into the orbit, and the release of the volume of the orbit to the front of the orbital rim can make the upper eyelid more hollow by “stealing” volume from the orbit. Fillers and fat injections into the lower eyelid region can improve this area in a natural way and is a great and better alternative in my opinion.
The YoungLift is a great way to rejuvenate the lower eyelid and cheek area. It is done with no incisions, with less recovery than traditional procedures and can be done without general anesthesia.

This is a question that I answered for someone who had a procedure by another physician.
This extra skin is a consequence of reductive type of surgeries. When you age, you lose volume and this contributes to the extra skin. Upper eyelifts /null take away skin and that usually leaves skin elsewhere. It won’t end. A better way to rejuvenate the upper eyelids is to volumize the whole area. This will require fat grafting and on occassion facial implants.
The extra skin between your eyebrows is a normal part of aging that can be accentuated with reductive type of procedures. This area may need to be filled in to plump up the deflated state. This will fill in some of the wrinkles and excess. There are doctors out there that can think outside of the box to give people a more younger and natural appearance. null is my exciting technique to volumize the face and its techniques can be used to fill in the area between the eyebrows, the forehead, and the temple area.

This is a question that I answered for someone who had a procedure by another physician.
Asymmetry can occur very commonly after upper and lower null or eyelifts. I may see that the left eye is slightly larger in the bottom part of it where the white is showing. This is sometimes due to the skin being taken which can cause contraction of the skin downwards making the eye larger appearing. The bags can be due to a number of things. When it is located right under the eyelid margin it is likely coming from the fat within your orbit of the eye. This fat can be easily removed. When it is lower it is likely due to the eye muscle that surrounds your eye socket and that helps you close your eyes that is causing the bag. This is called the malar crest and it usually is due to the eye muscle hanging over the smiling muscles.
Midface lifts can improve these malar crests but volumizing this whole area can improve the appearance of this cheek region. The larger appearing left eye can also be due to ptosis of the right eye or swelling. Ptosis is when the eyelid margin becomes droopy in relation to the iris. Ptosis can be temporary and can last months if caused by the procedure. Other times, the ptosis can be due to an actual injury to the muscle that opens up the eye (the muscle that pulls up the eyelid margin). This can require a more formal repair and is very difficult.

Rhinoplasty would be something that I would do first on the same day and follow up that with Fat Transfer. They can be done concurrently as long as you are ready for the extra recovery with all of those procedures. They are in different areas and will have a less of an effect on each other. Brow Lifting is another option to rejuvenate your eyes. But all you may need is fat grafting to subtly lift your eyes in a natural way and to rejuvenate both your upper and lower eyelids for the most natural and youthful effects. The null can be something that can help in a less invasive way. This technique entails volumizing the face with no incisions and is done through small puncture holes that heal essentially imperceptibly. Volumizing is the best way to hide eye bags without the hollow surgical look. The amazing part about the YoungLift is that you don’t need long incisions, general anesthesia (unless requested), and the recovery is often alot faster than traditional type of procedures. Traditionally, these under eyelid bags were removed through incisions under the eyelashes which often gave people a hollow and unhealthy look. The incisions were also more visible. Botox can prevent the muscles under the null from bunching and can reduce the wrinkles for a while. You can also get a laser peel there and will tighten up the skin in the area. A skin pinch can reduce the skin in this area also.
At age 19, a loss of volume from aging would be a little early for you. If this was the case, and you can find this out by looking at your pictures when you were younger to see if volume has decreased, you can add volume to the lower eyelid region with fillers and fat transfer. Given that you are so young most will likely suggest botox, skin care, sun protection. It would be better if you would send in pictures of yourself without smiling and you can tell how much volume you’ve lost in the lower eyelid area.

You can remove the bags under your eyes through a null through incisions under or inside your eyes. Skin can also be removed. This is the traditional method of doing this and improving the lower eyelid area.
You can also fill in the volume deficiency with fillers or some other volume enhancing method. Your lower eyelid has the baggy appearance often from the volume loss that occurs in the cheek area where the loss reveals the infraorbital rim. Replacing this volume can really enhance the appearance in this area with fillers. Fat injections may be the most natural way to improve this area and for a longer lasting effect.

This is a question that I answered for someone who had a procedure by another physician.
Otoplasty can lead to the sharp cartilages on your antihelix and can be due to a number of things. When you create the antihelix with sutures in otoplasty / ear reshaping for protruding ears / prominent ears you can alter the way the sutures are placed so that they make the antihelix broader or thinner.
When they are made thinner by putting the sutures closer together, they have a tendency to make the antihelix sharper and there is a point that they can bend and create sharp angles. Also, some techniques entail cutting the cartilage which can also lead to sharp angles and obvious shapes on the antihelix which I try to avoid. The nodules could be due to the sutures creating a reaction or if the sutures were to close to the skin they can create cysts.
These cysts sometimes need to be unroofed and expressed. Your revision will entail more effort to treat. You will likely need to harvest some of the cartilage within your ear to reshape the sharp angles through cartilage grafting.

I think when you set back your concha, sometimes you can create tension in the upper and lower parts of the ear and the possibility of a telephone deformity is greater (when the top and bottom of the ears stick out more than the middle portion.) It is effective for mild conchal prominences in my opinion.
If the movement that you are looking for is greater than 4-5 mm, a conchal setback my not be the best option. Greater movement and pinning requires excising parts of the concha. Then the decision is whether you should go through the front or the back. I’ve done many both ways. The back approach can be done without incisions that are visible in the front but you invariably have the possiblity of skin bunching up. Shaving the cartilage and weakening it can help move the whole ear and the bowl area back against the head and this technique works quite well.
When done well, the anterior approach (incision in front of the concha) can be hidden well and you have less bunching. You definitely want someone that has discussed all of the approaches with you. This will mean that they have had a lot of experience doing these most of the time. Knowledge is key and this is especially important in the area of surgery.
