Kenny Rogers and his Plastic Surgery

Here is a picture of Kenny Rogers. A lot of people think that he looks totally different and odd.  Kenny Rogers has apparently made comments that he is not happy with the work that was done.  I think the most significant part about his change in appearance is around his eyes.  His eyelift has been done in such a way that his crease is set too high for him.  With men you have to be careful in setting this crease too high. Prior to his surgery he had very low eyelid crease almost to the point of having asian looking eyes where their fold is low or not existent.  For men this fold should not be too high or they will have a feminizing look.  Ideally in women this fold should be about 1/2 an iris width high and in Kenny Rogers it is almost one iris width high which is too high in my opinion.  Also, he was supposed to have a facelift.  But I still see some neck banding which could be improved even more in this photo.

These things can be improved however.  His eyelid crease can be lowered through implanting some tissue above the crease and resetting the crease to a lower more masculine position.  His neck can be tightened a little more to help with the banding.  This would take some work but can be done so that he can look younger in a natural way!

Michael Jackson was an amazing performer. Did he have plastic surgery to his face?

Michael Jackson was a great performer and will be missed.  He was a part of everybody’s lives in one way or another and we will always remember him.  There has been much discussion in the past about his plastic surgery that he had done in his facial area.   During this sad time, I think its important to remember all the good things about Michael because he sure had a lot of them.  He brought a lot of things to people including myself. I really loved his music and he was always entertaining.

Latisse and Growing your Eyelashes

Latisse can grow your eyelashes tremendously!  You can use the medication and apply it topically on to the upper eyelid where the eyelashes are and they will grow over the course of a couple to several months.  Some of the main risks include worsening of your glaucoma if you have this.  Latisse can also make your eyelids darker where you apply it.  And if you get Latisse into your eyes it could make your iris, or the colored portion of your eye permanently darker.  So, if you have blue or light colored eyes this is something that you have to seriously consider that your iris will turn dark, or parts of it, permanently.

Can you nasolabial angle be changed or the way the nose relates with the upper lip?

This was a question a patient had regarding the relationship the nose was taking with the upper lip and how the angle was more obtuse. He wanted it more acute:

Yes, your nasolabial angle can get more acute after yourRhinoplasty. This can happen when you interrupt forces that hold your nasal tip up. The exposure that you do can also interrupt forces that hold your tip up. Trimming the top of the tip cartilages and interrupting the connection between the tip cartilages and the lateral cartilages higher in your nose can also cause your tip to descend and decrease the angle.

Also, interrupting the connection of your tip with your septum can also contribute to this drop in your tip and affect the nasolabial angle. The way to prevent this is less trimming of the tip cartilages and the usage of sutures to shape the tip. You also have to make sure to reconstruct the forces that hold the tip up through septal sutures that connect the tip with the septum. A graft in the area under your tip that extends from your tip to the top of your upper lip can increase the nasolabial angle and help maintain it.

Can rhinoplasty results be previewed before surgery?

Yes, you can alter the image of your nose through computer programs before a Rhinoplastyprocedure. This is a great way to convey to your surgeon what you want and for the surgeon to use as goals for the procedure. Although one thing to realize is that these are computer images and that the results cannot be guaranteed. You should be realistic about what can be done. Most plastic surgeons have this type of programs and you should ask for it because it helps. Some surgeons don’t believe in this and rely on pre and post procedure photographs to explain what can be done.

Rhinoplasty or nose reshaping just by working with the skin?

The question was whether you could just do surgery with the skin of the nose to change the appearance of the nose:

You can remove the skin around the sides of your nose by your nostrils to make them look less wide in this area. But if you want to make the tip smaller and the nasal bridge smaller, it will take a formal rhinoplasty. This at times will entail breaking the bones to narrow the bridge. Thicker skin near the tip area of your nose makes a little harder to narrow the nasal tip.  Some people reduce the skin from the inside to make the tip more refined, but this is not a safe procedure many times especially when your nose is done in an open fashion.

An open rhinoplasty entails making a incision under your nose to elevate the nasal skin envelope to directly work on the nasal structure under direct vision. Asian rhinoplasty / cosmetic surgery is unique and you should go to someone who has experience with the Asian nose.

Uneven nostrils and bridge too low after Rhinoplasty from another surgeon.

It’s still early after one month from a Rhinoplasty so you might want to wait a few months. I know this is hard, but a lot of times what you are looking at is swelling that is affecting the way it looks. Swelling can make a big difference.

Fillers for the bridge is a temporary solution that can cause problems. Radiesse is often the most considered for that region of the bridge but there are reports of complications with it there as well. Dr Toriumi, I believe has an article on that.

A rhinoplasty revision might be the best choice for you for your nasal bridge issue. If it is truly short, you might need some grafting material. Ear cartilage and septal cartilage can be used if it is a little deficient. But if you are looking for a big change like 3-4 mm of additional height you will need either a rib cartilage graft or silicone. Goretex is another option in this area.

The nostrils can definitely be different if things were not made even during the surgery.  The open technique is the most accurate means and your incision under your nose is very minimal. And, yes definitely you can bring pictures of yourself and your surgeon can help make it look a little more like what you had before. You’ll get the feeling of the right person to help you with this. You have to find someone that cares and will help you through this process every step of the way.

Rhinoplasty material for Asian Nose and whether Alloderm would be a good option?

Silicone is a good option, Alloderm will fade with time when consideringRhinoplasty for the Asian Nose. Silicone is the most commonly used. I have a friend that believes in Goretex in the nasal bridge. In my experience, Goretex is hard to make smooth but the mild integration of goretex can help fix it better in the nose.

Silicone has the tendency to move a little bit. But fixed well in the nose, the silicone can maintain a solid fixation to the bone. Silicone has the most experience in the nasal bridge over the years and has proven to be a great graft for the nose. It is really compatible with our tissue because it is right under carbon on the atomic chart.

Most of the time, silicone is placed in the dorsum through minimal incisions within the nose. My approach usually entails an incision under the tip in the columnelar area. I believe this cuts down on variables that can lead to asymmetry and uneven bridge although the recovery is more the results are more reliable. Alloderm does resorb and you won’t be happy after several months to years because it will go away. Also this material is taken from someone else and some people have issues with that with transmission of disease etc.

Rhinoplasty for a wide nose and with concurrent Acne and whether it is okay to do surgery with Acne?

Your nose can be definitely narrowed through Rhinoplasty and the Acne should not be a problem. Your acne should be under control before your surgery. There are many ways to control your acne through cleaning with a cleanser with salicylic acid, topical and oral antibiotics, a nightly retin A cream, etc.

For your nose, the width is ideally one iris width. Your nasal tip should also be one iris width. The base of your nose should be approximately 3 iris widths. When I mean an iris width it is the width of the colored portion of your eye. The narrowing will entail manipulating your nasal bones, and cartilage structures that dictate how wide your nose appears. The width at the base will entail taking some skin out around the base of your nose. This part can be done at a later time.

Choosing a surgeon with an eye for facial beauty is half the battle and more. For males, you may not want your nose to be perfectly ideal, as it could feminize your face. A wider nasal dorsum, tip and base is appropriate for the male nose and renders an element of masculinity.

Rhinoplasty for the fat nose

A wide nose can be reshaped through Rhinoplasty. Nose reshaping is one of the most complicated procedures in plastic surgery. The techniques used for the wide nose can vary depending on where the width is of concern.

The top third of the nose is made up of bone and the middle, cartilage and the lower third more cartilage but even more skin. There are different procedures for each area. The upper third requires manipulating and controlled breaking of the nasal bones in rhinoplasty. The middle third requires manipulating cartilage and part of the process of breaking the bones allows the middle third to become narrowed. The lower third requires or can require making the tip smaller and sometimes excising some skin on the sides of the nose can help make the base of the nose more narrow.

Sometimes, it is harder to make the nasal tip narrower if the cause is due to skin thickness. People try to overcome this by defatting the tip or adding grafts to shape it more. Consulting a surgeon who has extensive experience and a deep fundamental knowledge of rhinoplasty is a must for a great result. You want to go to someone with the best training.
