More on Brow lifts and the YoungVitalizer by Dr. Young of Bellevue | Seattle: This is a followup on browlifts done in a natural way.  As I have said before, aging is really a process of losing volume.  The loss of volume in the forehead, temple and around the eyes leads to the eyebrow drooping.  When someone comes in to see me, one question that I always ask is whether they want to look like how they were when they were younger, in terms of where the eyebrow sits, or if they want to look differently.  If a person wants their eyebrow to be more elevated then they ever were, than a browlift through surgery and incisions is probably the best thing for them.  If they want to look more like they were when they were younger then volumizing might be better suited for the person. If they want to look how they did when they were younger, volumizing around the eyes, under the eyebrow, above the eyebrow and in the forehead and temple can do amazing things.  The loss of volume in this area allows the brows to fall lower and also the loss of volume under the eyebrow and within the area aroung the eye allows the skin to deflate and this leads to extra skin around the eye.  Usually, historically, plastic surgeons tended to reduce the tissue in the face through facelifts, eyelifts, browlifts, etc.  When you age you change from a grape to a raisin.  Hence surgeons typically made the raisin into a smaller raisin. With this approach, you can look better but you don’t look like the grape and younger in a natural way.

One thing I realized in my approach to volumizing is that one of the most difficult areas to improve is the upper eyelid area and eyebrow area.  The face is essentially part of the whole head.  I know this is like a “duh” answer. But what I mean by this is much more complicated in a three dimensional view and from a beauty perspective.  So really it isn’t a “duh” subject, if you know what I mean.  The face has to be presented to the viewer, most importantly, from the frontal view.  Hence to make a face beautiful from the front, you need to present all the features so that the face is prominently presented to the viewer.  This objective has to be accomplished given that the face is attached to the head that eventually has to proceed backward towards the back of the head.  Why is that important? Well the face has to be volumized in order to push all the elements of the face forward so that when it is seen from the front it appears the best from this vantage point.  Hence volumizing needs to pay attention to this detail.  When one volumizes the face, the lateral parts of the face needs to have more volume to push those elements forward so that from the front they occupy a prominence to present the face beautifully to someone seeing them from the frontal view.

A big question is then presented to surgeons like me.  If we volumize symmetrically in all the areas of the face, won’t you recreate what the person had when they were younger. This could be true so that you wouldn’t have to volumize more of the lateral areas of the face.  But this all depends on the particular person and how they were built.  You see, sometimes beauty is created by soft tissue or hard tissue. If it so happens that a particular person’s beauty is created by hard tissue like bone, they are more likely to hold on to their beauty.  My wife’s family is like that.  Their cheeks are made of bone and not soft tissue.  So you have to determine how much of there bone structure is still present.

How does this apply to browlifting and rejuvenating the eye region? Well you have to determine how their eyebrow is currently shaped and how they were when they were younger and what volume will get them to the past.  I usually find that most people require much more augmentation in the lateral part of the brow. But typically I augment the lateral brow 2-3 times more than the medial part of the brow.  When I volumize I usually also have the tendency to augment so as to lift the eyebrows with more of the augmentation under the brow and above the eyebrow.  See below for an example of augmenting the whole face and especially in relation to this post the eyebrow and the upper eyelid region.  “The YoungVitalizer” procedure was done for this patient below.

In terms of a browlift (or brow lift / eyebrow lift / eye brow lift / foreheadplasty / browplasty) you can fill in certain areas of the face to lift the brows. One thing to remember is that with volumizing tissues tend to follow areas that are volumized.  For example, volumizing the cheek next to the inferior and laterally next to the eye, tissues will tend to follow towards that portion of the cheek.   When that cheek receives more volume, the nasolabial folds and jowls began to migrate toward that lateral cheek area.  This same principle applies the eyebrow.  To elevate the lateral portion of the brow, the area above and lateral to the brow will need to be filled.  Take a look at some pictures of beautiful people and you will notice that there is an area above and lateral to the eye and brow that receives a lot of highlights from whatever light source that particular face is located under.  Hence to lift the brow you need to place tissue above and lateral to the eyebrow in the forehead and temple region.  One thing to realize is that augmenting close to bone serves as a foundation to have tissue migrate towards the volumized area.  This foundation needs to dominate the filling.  If you want the eyebrow to lift you need to place the tissue above and lateral.  If you place too much tissue in the eyebrow itself, this filling can dominate and prevent the eyebrow from migrating to where you want.  Also when you augment next to bone below the eyebrow in excess the eyebrow will tend to migrate inferiorly.  All this takes skill to master and the more experience in volumizing the better.

This blog discusses our name change from the YoungLift to “The YoungVolumizer”.  We had to change the name for various reasons.  My wife actually came up with the name YoungVolumizer.  I was thinking about the name for a while and when she told me her idea of the YoungVolumizer I instantly liked it.  I then discussed it with the rest of my staff and they felt that it was also a great name.  We are making people look younger by volumizing their face. We hope our readers and followers like this new name.

Will fat grafting make me look fat and can you do fat grafting in the lower eyelid with a lower eyelid blepharoplasty?

Fat injections (fat filler, fat transfer, fat grafting, pearl fat grafting) done right will not make you look fat just younger. Repeating the lower eyelid blepharoplasty and doing fat grafting is not a familiar course for me. I either do one or the other and I tend to do more fat grafting.  I only consider the lower blepharoplasty with fat grafting when there is an extraordinary amount of fat in the lower eyelids and this is not common.  Most of the time when you remove fat and skin from the lower eyelids you tend to look hollow there.  Fat grafting as a whole can really help this area by bringing back the volume to the lower eyelid and upper cheek area.  Also combining the two procedures can have an impact on the fat survival in my experience.  With fat grafting, I always have patients bring in photos of themselves when they were between 10-20 years old.  This helps me to determine where to put the fat.  Fat grafting will not necessarily make you look fat at all. When placed correctly, it can dramatically make you look younger.You just need to put the fat in the right places. Also browlifting is more complicated than most surgeons think.  A little can do a lot to make someone look surprised.  Many times fat grafting around the eyes, temple, and forehead can really make someone look younger in this area while elevating the eyebrows or giving the illusion that it has been elevated. My right eyebrow is higher than my left. Is there a subtle way to correct this?

This is a question I recently answered for someone:

Asymmetry in the eye area is normal.  In fact, we appreciate beauty in an asymmetric way.  Our right brains appreciate beauty more than our left and our visual fields are asymmetric.  Essentialy, in short, we see someone’s right face more when we regard or assess beauty.  This has evolutionarily lead to asymmetries in our face based on millions of years of sexual selection.  We tend to choose our mates based on how good the right sides of our face look more than what our left sides look.  Thats sort of the long story and there is even a longer explanation to that.

In terms of correcting, you should realize that not everybody is the same on both sides.  This may comfort you a little and might dissuade you from getting something done.  What I can say is that you can get some correction of this if you are willing to do this. From this picture, I would really have to see you in person or see pictures of you to really know, but your left eyebrow is a little lower.  You might benefit from adding volume to the left temple region and above the eyebrow to lift the eyebrow subtly.  Even adding some volume below the left eyebrow could benefit.  This can be done with Fat Transfer, Facial Fillers, and other implants.  A browlift could even pull up the left eyebrow but in a less than natural way.

Fillers, Botox or Lasers for the Under Lower Eyelid Area?

This was a question I answered for someone recently:

Restylane is a filler and can be used to fill in the lost volume that is really the cause of the majority of the reason why your lower eyelid looks the way it does. Fat injections, the YoungLift, Juvederm, Perlane, Radiesse are other fillers that can be filled into the lower eyelid area.  The temporary fillers last from 6 months (restylane) to a year or more (radiesse) with perlane and juvederm in between.  Fat injections can last much longer but this isn’t guaranteed but is the best shot for a long term correction.

Botox prevents the muscle around the eyes from squinting which can make the wrinkles less noticeable. Dysport (or botox / bo tox / botulinum / neurotoxin / disport) is a great option.  But these do nothing for the surface quality of the skin and neither does the filler. But the fillers can decrease the amount of wrinkles by filling up the volume.

This is where the co2 laser or other resurfacing procedure (erbium yag, active fx, ultrapulse, deep fx, max fx, total fx, traditional co2 laser resurfacing, laser peel) can help with by decreasing the wrinkles and actually tightening the skin.  A small amount of volumizing also occurs with lasers with their creating of a thin collagen layer deep to the skin.

What is the best to replace volume in the upper cheeks filler or fat injections?

Both are good options with positives and negatives. Facial Fillersused for the upper cheeks include radiesse, restylane, juvederm, perlane. These are temporary. Radiesse should only be used in the deeper planes of the cheek especially around the lower eyelids.  The white appearance of radiesse can be seen through the thin skin of the eyelids if you are not careful and place it too superficial. The others are hyaluronic acids and they are clear looking and can be place almost anywhere.  Because they are clear when they are injected into areas of thin skin, your skin can take on a translucent look and sometimes this can look like a very light bruise.  They are fairly easy to put in and you can get relatively quick results but they are temporary lasting anywhere from 6 months to a year or more. Restylane lasts about 6-9mo, juvederm maybe a little longer but it comes supplied with less material 8cc compared to 1.1cc of restylane. Radiesse lasts about a year sometimes more or less.

Fat injections however can last for many years.  Usually you can get approximately 60-90% of the fat to survive depending on how well you do the procedure. Because it is a living tissue technique is vital to insure survival of the fat.  When carefully placed fat is the most naturally looking of all filling or volumizing type of procedures.

Fat is taken from another part of the body and transferred to an area that needs volume

Fat transferring entails taking fat from some area of the body where fat is not wanted and then transferred / injected into an area that is needed or where volume is needed.  Usually very small puncture holes are needed to achieve great results.  This procedure is highly technique dependent.  Proper technique can make the results last for years.  Attention to detail is a must.

What are the options for reducing the size of your pores. Does a facelift work for this?

This is a question that I answered for a client who had questions regarding whether a facelift would work to reduce the pore size as well as acne scars:

Fat grafting, laser resurfacing, ipl are better options than a facelift (face lift/ lifestyle lift / quick lift / lunch time lift / mini lift / weekend lift / s lift / lifelift / thread lift ). Some newer studies and results from physicians more recently are showing that these modalities may be more beneficial to reduce pore size for people.  A facelift will not effectively do this but it can improve your wrinkles to an extent.  Fat grafting (or fat transfer / fat augmentation / facial fat transfer / fat injections) has been shown by some to regenerate the skin including reducing the size of your pores.  Stem cells in the transferred fat can rejuvenate the skin for years.  Laser resurfacing through the creation of new collagen can also reduce the size of the pores as well.  Ipl is another newer treatment for pore size.  Out of all the options fat grafting might be the best option for pore size and acne scarring.  More and more surgeons are using fat grafting as a way to improve acne scarring.

What are some options for filling in dents in the cheek?

Fillers, the YoungLift, Fat Injections, Cheek Implants are options for those areas.  Having experience in augmenting areas such as this is important.  Facial Fillersare temporary (restylane, juvederm, perlane, cosmoderm) but can be a way to fill in those dents and a good way to assess whether volume can help.  Fat injections are a long term possibility to augment that area.  If the fat survives after 6 months it is likely to last for years.
