Question: What can be done for reoccuring jowls after having a facelift 3 years ago? What about the thread lift?

Answer: A Chin Implant Reshaping or Fillers / Fat injections (Dr Philip Young Seattle Washington)could help the looseness in the jowls.  The jowls occur for a number of different reasons. If you read my other blogs you can find pictures that illustrate jowls.  What happens is that when you lose volume in the face, especially the cheeks and jawline area along with volume around the mouth, the jowls become more prominent.  As an example, pinch the skin by your upper cheeks just inferior and lateral to the eye.  Notice as you do this, that the jowls begin to rise a little bit.  Now in front of your jowls is a depression and volume loss call the prejowl area.  This area along the jawline and closer to the lips and in front of the marionette lines can be filled in as well.  Also along the jaw in front of the ear can also be filled in and this can have an effect on the jowls decreasing.  Sometimes people fill in the prejowl area with fillers and fat injections to reduce the appearance of the jowls through a camouflage type of correction. In terms of volumizing with fillers, you can also use fat injections for the same thing and you can add more volume with fat than you can, a lot of times, than with filler.  Fillers are quite a bit more expensive than fat for filling in volumes.  Sculptra is another option in between fat and fillers. Sculptra can last over 2 years and some are finding more than that.  Other options for the jowls include direct liposuction of the jowl area.  Thread lifts, in my opinion, don’t work. They pull on the skin for a time being but relax and most of the time I don’t think you get any long term improvement.  The only time you get long term improvement is when you turn the thread lift into basically a facelift done in many traditional ways. How do you get naturally looking lips and what are some tips for natural looking lip plumping?

Aesthetics are key to understanding the lips and how to make them beautiful through lip augmentation (Dr Young Lip Enhancement in his Bellevue Office). Aesthetics in the face is really understanding facial beauty and this is central to the process of making one’s face and lips beautiful.  Previous theories do not have a good explanation for how a lip should appear in its most beautiful state.  I would invite you to visit my blog and website to see more answers to this.  But I discovered a new theory on facial aesthetics called the Circles of Prominence that I think answers some of these questions. To me the lip is assessed by the highlights that it creates. If you look at someone next to you, you will notice that all the light hits the lower lips.  This is very important.  You retina is highly sensitive to light and dark.  The highlights that the lower lip creates attracts a lot of attention and it is the lower lip that is the center of attention in the mouth area. The lower lip should be the same height as the iris and the puckering that it creates should be 3 iris widths.  Essentiially the lip emulates the shape of the eye, usually in reverse form.  The lower lip should be half the height of the lower lip and the puckering of the upper lip should be centered and should be one iris width in width and again one half an iris width in height.  The goals of augmentation should be to reach this.  This is the reason that many hollywood people who get their upper lips too big start to look odd.  You don’t know why inherently, but your brain does.  And the reasons I stipulated above explains why your brain feels the way it does.  Whether you use temporary fillers, like restylane, juvederm, perlane or more permanent options.  These aesthetic goals you should direct your surgeon in how to augment your lips.  More permanent options include fat injections, vy advancements, lip lifts, or silicone solid implants, like surgisil, or permalip.

Fillers and Fat injections for Cheek Augmentation by Dr. Philip Young of Bellevue | Seattle:

Fillers Upper Lower Eyelid Eyelift

Radiesse is made of Calcium Hydroxyapatite which is the exact molecule that is like our bones except without the cells and other tissues like collagen, blood vessels etc.  It is very compatible with our bodies.  Studies done by Radiesse have shown that this product is replaced a little bit by collagen.  Radiesse has been shown to be present after 5 years by biopsy and some of this work has been done in the area of urology and the bladder neck.  It is a stiffer products than the other fillers like restylane, and sculptra.  The product is made up of 30% calcium hydroxyapatite so the rest of the 70% is removed over the first 6 months to a year and the 30% can stay around longer.  Some people seem to metabolize the calcium more quickly and this can decrease the longevity, although this is through my observation and has no scientific analysis to it yet.

Sculptra is made up of poly l lactic acid and is like vicryl suture which is degraded by our body to water and sugar which we can use.  The degradation process is what produces the inflammation that eventually creates collagen, in a controlled scar like fashion.  Results are for at least 2 years and they are finding that it lasts even longer in many instances.  I have a patients whose results have been there for over 5 years.  This inflammation is beneficial in that it produces collagen but on the flip side this inflammation when not controlled can lead to infection although this is very rare.  Although most of the doctors mention bumps, this can be avoided.  I have some techniques that make this much less likely.

Fat injections are a great longer term option.  They are natural. They take on the structure and feel of the tissue they are placed next to.  If they are placed next to bone they will feel like bone and etc.  They can last years, although this depends on the particular person, some results can last 10 years or more. They require no incisions in the face, the discomfort is mucher lower than traditional approaches like facelifts, eyelifts.  They do require a recovery period from the bruising and swelling but is acceptable.  Unlike sculptra and radiesse, fat can be placed closer to the eyelid and lashes.  Whereas these products need to be along the bone under the eye and this is where you can some problems with transitioning the volume from the bone to the eyelid whereas fat can do this transition. In terms of transitioning the product to the eyelid wiith radiesse and sculptra, the other alternative is to do transitioning and touchups with restylane, perlane or juvederm when needed. These hyaluronic acids (restylane, perlane, juvederm) can also be used for cheek augmentation but last less long (6 months to a year) than radiesse (year or longer) and sculptra (at least 2 years).

What can you use to fill in the hollows of the face?

Fillers and Fat injections (Dr Young does Filler in his Bellevue Office!) can fill in the hollows.  Fillers like restylane, radiesse, perlane, juvederm, can fill in small hollows that are smaller than 1cm squared or  a square area about a 1/3 of an inch.  Fat injections are another more longer term possibility.  Sculptra is a filler made up of suture material that when digested by the body forms collagen for a more longer term solution as well.  The YoungVolumizer is my method of volumizing the face and hollows.  I have a great technique that leads to better fat survival and more long term results.

The mouth area undergoes a lot of changes that can contribute to nasolabial folds, marionette lines, smoker lip lines and a downturned mouth.  Volume loss plays a major role in these changes.  Replacing this volume can have a profound impact in rejuvenating the aging mouth and the YoungVitalizer is our approach to improving this area.  Fat injections are a great way to do this and other fillers can help as well.  Sculptra can help if fat is not available.  Fat and Sculptra are good options for permanent or semi-permanent effects.  By this, I mean results that last more than 1 year. The picture below shows the locations of the nasolabial folds, marionette lines, and prejowl volume loss.  All of these issues can find their origination from volume loss to a large degree.  For the nasolabial folds, there are studies that show specific fat pockets that when re volumized can play a major role in eliminating nasolabial folds.  Specifically the area lateral to the nose and immediately deep to the most medial and superior area of the nasolabial folds is the area where re volumizing can play a big role in reversing nasolabial folds.  Furthermore volumizing in the whole area inside the nasolabial folds and marionette lines can rejuvenate the peri oral and mouth area.  Volumizing in the area immediately medial to the marionette lines can diminish these lines.  Volume in the prejowl area will blend this area in a more pleasing way with the jowls and in turn make them seem less apparent.  This is in line with the whole idea of doing prejowl implants with facelifts to improve the jawline.  Also volumizing the chin can restore this whole area as well and give support for the other areas that are volumizing.  One important thing that I have learned is that volumizing other adjacent areas can help support other areas by aiding in the volume expansion.   An example is if you place on stake in the ground and place a very heavy tarp over this stake.  It is more likely that you will form a nice area under that tarp the more stakes you put under the tarp.  It is also more helpful the more stakes you spread out in the area to give the original stake support.  Volumizing the chin is analogous to this idea.  When you volumize the chin it supports the volume you put in the prejowl area as well.  More likely than not your chin has lost volume as well and volumizing deep to the chin will rejuvenate your lower third or lower mouth area.  Also people are likely to develop deep wrinkles in the labio mental sulcus which is the crease that is under the lower lip.  Volumizing deep in the chin just inferior to this crease will help to eliminate this crease.

How do you get rid of the Angry Look?

How do you get rid of the Angry Look?  But what if you are one that doesn’t want to come in for Botox several times in a year to keep the results. Botox is great, but a browlift or volumizing followed by laser resurfacing are options.  A browlift that pulls up our eyebrows and then with additional work to take out the muscles that are causing that frowning can be an option to improve this furrow.  I think volumizing this area can also be a good option.  Loss of volume in the forehead and in between the eyes can lead to more skin relative to the tissues underneath. This leads to more skin folding and a closer position of the skin to the muscles that are creating the wrinkles.  Volumizing takes up the extra skin and also separates the skin from the muscles more and decreases the muscles effect on the skin, ie wirnkles.  After both a volumizing procedure and a browlift either/or, you can resurface the face to decrease the wrinkles and also to subtly shrink the skin. After a facelift you continue to age but you will always look better than a twin that never had it done by Dr. Young of Bellevue: After a facelift, you continue to age but you will always look better than a twin that never had it done.  Of course, you have to make sure a facelift is what is going to make you look good.  I think that when facelifts are redone more than once, the more you do the more you can look distorted.  When you age with a facelift, some have found that you can have what people call the lateral sweep affect.  This occurs when your face is pulled laterally and what keeps it up is also more lateral and in between the tissues can hang as you age.  What you get is two points that are higher, one lateral and one nearer to the midline of the face creating a Nike type of swoosh appearance.  At some point, you really need volumizing to make your face look younger.  So if a person has had a facelift before and wants to look younger, I usually talk about the YoungVitalizer as an alternative.  This is my technique of volumizing the face.

Surgery with a Facelift or volumizing. What should I do? by Dr. Philip Young of Bellevue | Seattle: I think ultimately when you talk about facial rejuvenation, at one point you will face this question of whether to undergo surgery to remove extra skin and lift or the other choice of filling up the space to volumize the extra skin and the space around it.  Take for example the eyes.  Traditionally plastic surgery and its surgeons typically took away skin and fat.  That usually improved the situation but often times, in many cases, the person didn’t look younger necessarily.  Aging is predominately a process of losing volume in the face and around the eys and the way to reverse that is to replace this volume.  Here is a pictures of what improvements you can get after volumizing around the eyes: The YoungVitalizer.  I usually say that aging is a process that is analogous of a grape changing into a raisin.  Traditionally plastic surgery would make that raisin into a smaller raisin and not like the younger grape that it once was.  Volumizing returns the person’s face back into the grape it once was.

So when do you opt for the traditional reductive type of procedures where things are removed such as when you do an eyelift, browlift, facelift and the like? Well it depends on how you looked like when you were young.  If you thought your face was larger than you liked when you were young then some reduction may be necessary. I have a lot of Asian women who used to have much larger faces which they didn’t like.  They like there shape now that it is smaller but they still look aged.  I think some reductive type of procedures for this particular situation may be more necessary.    Reductive type of surgeries are also more indicated when the person gains a lot of weight and changes the face shape from what it was when that person was younger.  In this case as well, reductive type of surgeries would be more beneficial before volumizing.  Ultimately, whatever the face is looking like right now and what changes it needs to reach the ideal is what you would be most beneficial in doing.  I use my theory to find that ideal for people.

Nasolabial Fold Grafts are another option to improve the Nasolabial folds by Dr. Philip Young of Bellevue | Seattle: Nasolabial folds are a very common structure that develops as one ages and it is one of the most common structures that people want to get rid of.  I’ve learned a lot of things about improving this structure.  Facelifts have been experimented with over the years and most people believe that the deep plane facelift when the SMAS is lifted over the smiling muscles is the best way to improve the nasolabial folds from a facelift prospective.  Others find that a midface lift does this better.  Others find that volumizing the cheek in certain ways improves the fold as well.  Fillers have been used in the nasolabial folds to improve them and this is a very common option that people have chosen. Here is a video explaining the  differences between deep place facelifts and mini facelifts:

My opinion is dependent on the person’s anatomy.  If the person has a lot of tissue and has some extra weight that they have gained.  Volumizing might make the face a little too big.  In this case a facelift can help improve the nasolabial folds.  One thing that can help the folds is too fill the volume up with a nasolabial fold graft.  This graft can be taken from the SMAS (superficial muscular aponeurosis system) which is the layer of tissue that is connected to all of the facial muscles. The SMAS is convenient because this the layer that the facelift uses to pull the face and can be easily taken to use for the nasolabial fold graft.  This graft can be placed in the nasolabial folds through the nose and can help improve their appearance along with the deep plane facelift.

If a person is against such a surgical procedure than I think volumizing might be the best choice for them.  This can be done with essentially no incisions.  The YoungVolumizer is great for this.  If undergoing a procedure like this is too involved for the particular person, then volumizing with fillers such as restylane, radiesse, juvederm, and perlane are options.  Sculptra is another long term permanent filler option that can also be used to improve and volumize the face to improve the nasolabial folds.  In terms of fillers and volumizing, there is a special location, an area that contains a deep fat pocket that really helps to decrease the nasolabial folds when it is volumized.  It takes a lot of experience to learn how to fill up this fat pocket with fillers and other volumizers, like the “The YoungVitalizer” procedure.

Reducing forehead wrinkles

The aging that occurs in the forehead entails a volume loss and it also entails some loosening of tissue and ligaments that hold structures in place.  As the eyebrow descends, we tend to elevate them to improve our vision and this can lead to wrinkling in the forehead area. This situation is coupled by our loss of volume (fat, collagen, bone loss).  This loss of volume places the vicinity of our skin closer to our muscles in our face.  Hence when we move our muscles in our face, like our forehead muscles, the muscles exert more action on our skin just by the fact that they are closer to the skin.   Through volume we can create a distance between the muscles and our skin and hence decrease the influence of the muscle on creating wrinkles. The question is then does the volume make us look weird.  The answer is simply “no”. When we were young, we had more volume all throughout our face.  If you take a look at pictures of yourself when you were young, you will notice a lot of loss of volume in your face.  Take a look at your temples, forehead, and your cheeks.  It will take a little time to adjust your thinking but you will see the difference. This loss of volume places your muscles closer to your skin. This is one of the main reason for forming wrinkles.  The amazing thing is that through volume, you can actually affect your skin change it for the better.  With the volume, the skin is allowed to remodel itself.  Your skin is in fact always turning over the cells and tissue within its structure.  With the decreased influence of the facial muscles on the skin, the cells within your skin begin to remodel and decrease the wrinkles that it has created all of those years.  This process of skin rejuvenation after volumizing can take many years and the change will be gradual.  This is a couple of reasons why the YoungVitalizer is my procedure of choice for facial rejuvenation.

In terms of the forehead, the volume can elevate your eyebrows to a state that you were a long time ago.  The volume again will also improve your wrinkles as well.  Volumizing is the most natural way to rejuvenate your forehead.  When I do forehead rejuvenation and rejuvenation of the upper eyes, I place the volume deep below the eyebrows, inferior to the eyebrow and within the upper eyelid, above the eyebrows and also in the temple and forehead areas.  This volume helps lift the eyebrows and can brighten the appearance of the eyes better than any other form of facial rejuvenation.

Browlifting is another way to do so.  You can elevate your brows and this can decrease your need to lift your eyebrows and ultimately your wrinkles can improve.  Also through browlifting some of your muscles can be altered to weaken them.  This is useful in the area in between your eyebrows and eyes.  Browlifting however can lead to less than a natural appearance.  Your eyebrows can be elevated to a point that is higher than they use to be. Also the removal of the muscles in between your eyes, although it will help decrease the movement there, the volume loss can make this area look worse.  If you want your eyebrows higher than they used to be, a browlift might be the better option.  But this is probably the only option that is better than volumizing in my opinion.
