The tip of the nose can be reduced with local anesthesia without siginificant problems when someone undergoes nose reshaping (nose job / rhinoplasty / cosmetic surgery on the nose / nose shaping / rhino plasty / or rino plasty). You don’t need to be under iv sedation or general anesthesia for reducing the tip.  The tip is easily made numb for tip rhinoplasty.  The tip can be reduced by altering the  cartilages through an incision under the nose.

Rib and Ear cartilage are the most natural ways /sources to increase the height of your nasal bridge during rhinoplasty / nose reshaping (other names for this procedure include nose reshaping / nose shaping / cosmetic surgery on the nose / rhino plasty / rino plasty / nose plastic surgery). Rib cartilage has the advantage of having an abundant supply of cartilage. Their is a tendency to warp and this can be prevented by placing pins inside the rib or by equal carving on all sides of the graft. There are other ways to prevent the warping as well. Ear tends to get absorbed more than rib and also is not as resilient and has a natural bend to it. It really depend on the person whether ear or rib hurts more. In my experience, rib grafting is far more painful than ear harvesting. One option for nasal bridge augmentation is using silicone implants. They are very compatible to our tissues and are almost the ideal implant. You don’t need an extra place to harvest it and can last for as long as you live without absorption, warping, and the need for another area to harvest. The argument against silicone is that is not natural and can lead to infection but there are data out there that shows the rib cartilage can have just as high of a rate for infection during rhinoplasty / nose shaping / nose jobs.

what type of minor procedure can be done for the small hump on the nasal bridge instead of rhinoplasty

But these solutions are more temporary and last anywhere from 6 months (Restylane) to a year or more (Radiesse). Fat injections are another solution to augment the nose to hide the bump (or nasal hump / nasal bump / hook nose / prominent nose / large nasal bridge / nasal bridge hump).  When I talk about these injections I mean injecting the fillers above and below the bump to hide it better. This type of procedure can be done in the office under minor local anesthesia with minimal bruising and swelling that lasts only a couple of days versus a week or more for rhinoplasty.

ARhinoplasty (or nose shaping / nose reshaping / nose plastic surgery / nose cosmetic surgery) is essentially any procedure that alters the shape of your nose and that can mean what I just described to the more traditional thought of during a more invasive surgical procedure to change the shape of the nose. A rhinoplasty has been called a nose job or referred to as nose reshaping / shaping, etc.  From a more traditional standpoint, taking down a hump will entail making small incisions within the nose to file down the bump.  Sometimes in order to make the nose appear more normal, you need to cause controlled breaks in the nasal bones to make it close in after the filing takes down the hump.  This type of approach is more longer lasting but does take a little while longer to heal versus the filling techniques that I described above.

How long does Alar base reduction results take to appear

This was a question that I answered for a person who had a procedure done somewhere else by another physician:

Rhinoplasty during a rhinoplasty / nose shaping / nose reshaping / nose job procedure are done for big nostrils, wide nose near the nostrils, or big noses in general.  The area around the nostrils are called the alae and alar reduction or augmentation is used to change the shape of the alae. Alar reduction results should be earlier than later compared to other results with rhinoplasty / nasal reshaping / nose job  such as tip reshaping / tip rhinoplasty or modifications for a bulbous, big nasal tip. Alar base reduction swelling and the results should be faster when compared to work done on the tip area.  Generally, healing takes place on a logarithmic scale. Simply put, 60% of your healing should take place the first 6 weeks, 80% at 6 months and 89% at 2 years.  You will never have 100% of the strength that your tissues had before the procedure.  So in general, by 6 months 80% of the results should be present.  This is a generality and this is important to understand though. The above percentages are based on wound breakage strength and many surgeons and physicians use this to assess how healing in general takes place.  Usually results with procedures that are done in the tip take a while to be realized while other areas are a little faster like your nasal bridge and work done around the alae.  Your physician should best direct you.  But a second opinion never hurts in my opinion.

Stephanie Pratt and her rhinoplasty/ nose job?

From the looks of the picture, it appears that Stephanie Pratt has some changes to her nose which could very well be from a Rhinoplasty.  From my eye, her nasal tip looks significantly different.  The tip is rotated higher and it is smaller in size. Also the upper portions of the nasal tip has been made smaller as well. Her bulbous tip has been refined so that is is closer to the size of her iris.  It is a really good job done by her plastic surgeon.   She does have a narrowing in the area above her nasal tip that could have been improved to a little larger width like the rest of her nose and hence have more harmony for the entire nasal bridge.  Based on my theory on facial beauty called the Facial Beautyher tip is near ideal size and shape!

Heidi Montag Nose Job, did she have one and how are the results

heidi-montag-breast-implants This is a picture of Heidi Montag. Because I specialize in the face and neck plastic surgery and not the rest of the body. I will comment only on what I see in her face.  From the look of these pictures, it appears that Heidi has a change in her appearance from the left to the right.  Her nasal tip looks to be more refined with less of a bulbous appearance.  This is a common maneuver in Rhinoplasty.  The ideal size of the nasal tip should be about one iris width in dimension.  Anything bigger will attract more negative attention because it draws more attention to it than the iris.  When it is the same size as the iris, the nasal tip harmonizes with the iris and there is a connection between the nose and the eye.  When the nasal tip is smaller than an iris width, it attracts less attention and is less of a distraction but it harmonizes less with the iris and hence will be considered less beautiful.  Based on these photos, if she where to have had a rhinoplasty, I think the results have definitely improved her Facial Beauty!

After rhinoplasty from another surgeon my nostrils are bigger after nose picking and is this possible.

This is unlikely unless your nostrils were made to be smaller with suture techniques during Rhinoplasty.  If this were the case, your nose picking could have dislodged or broken some key sutures that were keeping your nose a certain shape. Also if your nostrils were made smaller from the surgery, your nose picking could have stretched out the reshapened nostrils and made them smaller.  Making your nostrils bigger is actually less of a concern than if you were to tell me that your nose is smaller after picking your nose. Contractures can happen that can make your nostril smaller and the correction of this is much more involved requiring grafts to enlarge the nostril and sometimes the need for multiple revisions to enlarge the nostril.

There are many reasons to lead to a shortened nose throughRhinoplasty.  Suturing techniques are the easiest to reverse and you CAN cut some sutures to make it lengthen again.  However if the shortening is due to cutting the septal cartilage the treatment to lengthen this situation is more involved and may need cartilage grafts that are situated in an extension type configuration (often called septal extension grafts)in order to lengthen the nose.  Your surgeon will really know how to do this for you. But if you were to get another surgeon to help you with this you could have your operative note sent to the new surgeon.  Don’t things can be done. You will look better!

Radiesse injections to the nasal bridge and how long to wait before rhinoplasty

If the augmentation is lost, I think it would be okay to consider Rhinoplasty. Facial Fillersis made up of 30% hydroxyapatite and 70% of the carrier. Much of the carrier dissipates over the course of the year.  It might be prudent to wait. But if you think the results are essentially back to normal you could consider rhinoplasty and an implant to raise your nasal bridge. What is left in your nose is likely the hydroxyapatite and this could stay in your nose for years. Biopsies of the bladder have shown it to be there for 5 years.  Hence if most of the augmentation is gone you could do the rhinoplasty earlier than the full year that it is expected to give you results.  If you really wanted to be safe you could wait for the full year to pass.

Best implant in the nose Goretex, silicone for the Asian Patient

Goretex implants for Asian Plastic Surgeryare usually made out of sheets and are formed by the surgeon to fit the nasal characteristics of the patient.  Goretex is made up of fibrillated polymer of polytetrafluoroethylene with pore sizes in the range of 22um which allow limited tissue in growth. It has been determined that pore sizes between 1 and 50 um (micrometers) will allow bacteria to get into the pores but not tissue ingrowth to fight off the bacteria.  Hence pore sizes between 1-50 um are not supposed to be good.  But there are a lot of surgeons that have used gore tex in the nose without a lot of problems and it has a proven track record in the nose.  It is a soft implant and allows the tissue to fixate the implant for a more stable feel.  Theoretically the chance of extrusion or rejection of these implants versus silicone should be higher and some studies show that is the case but the chances of this is still pretty low.  I have a good colleague that is very well known and totally believes this is the best implant to use in the nose.  I however based on experience and knowledge of different products believe that silicone is the best implant for the nose during Rhinoplasty.  It is the most compatible of all implants and has a lot of characteristics that come close to the ideal implant.  It allows limited tissue ingrowth but is very stable. Some people believe that the implants allow to much movement, are not as stable, and can look unnatural.  There are ways to fix the implant to the deeper structures that make the implant feel as if it were bone in certain places such as in the nasal dorsum/ bridge.  The silicone implant can also be placed within the tip in a judicious way to augment the tip, raise it and shape it with great success.  The alternative to this is to harvest cartilage from the septum or ear with the drawbacks of these areas. Septal cartilage is sometimes sparse in the asian nose and ear cartilage is often times too flimsy and curvy for the nose. Rib cartilage is sometimes the best natural alternative for the bridge and all the other areas an Asian nose might need.
