Swelling 7 months after rhinoplasty and what is going on with that after a procedure done by another surgeon.

This is a question that I answered for another patient who had rhinoplasty surgery somewhere else.

Your skin has memory of its own especially if it has had previous surgery. The scar tissue from previous work can cause the skin to take on its own shape. But ultimately the cartilage that you have in there will dictate how the final shape will be. The work your surgeon does with the cartilage is the most important and your surgeon will know best how that will determine the ultimate appearance.

Swelling can take months and up to a year for the results to show more clearly.  It is not out of the ordinary that you will continue to have changes up to two years after your procedure. Steroids after the surgery can help with swelling as well as bleeding precautions. Taping of the nose can have an impact as well and your own care of the procedure afterward Rhinoplasty can be very important.

Eventually the skin will shrink down to the cartilage level and will take on the shape of the cartilage structure that was created!

You have a nasal bridge hump that is likely made up of bone and cartilage. These can be taken down incrementally so that it can look the way you want from the side with a Rhinoplasty. This change can be done so that your view from the front is essentially the same. The lighting on your nose will be slightly different but it will be better because the light on your nasal bridge down to your nasal tip will be more congruent and similar to indicate that the bridge is one whole structure.

Currently, your tip will display different highlights that will make your bridge look non continuous and consisting of different parts. But this procedure can be done and you will look super!

Fat grafting to the lower eyelid, upper eyelift and rhinoplasty at the same time

Rhinoplasty would be something that I would do first on the same day and follow up that with fat grafting. They can be done concurrently as long as you are ready for the extra recovery with all of those procedures. They are in different areas and will have a less of an effect on each other. What to do about the scabs inside the nostril after rhinoplasty?

The hydrogen peroxide can help break up the crusts inside your nose after Rhinoplasty. But you could also be doing some nasal saline rinses to keep things moist and allow the crusting to break down. Your surgeon can also help take some of the crusting away with instruments. Sometimes depending on what was done, your airway may be too open and this can cause a dry nose syndrome where the inspired air is too easily passing through your nose causing it to dry and also possibly get infected. Your surgeon could help in determining this possibility.

After care advice for rhinoplasty surgery?

Your Doctor should have an after care packet for you to follow after your Rhinoplasty Make sure to read all of the material and ask questions.  Also you should do your own research to learn more on the internet.

Your doctor can only supply you with a limited amount of information and everybody’s fund of knowledge is different and some may need more supplementation than others.  In general, the less trauma the less scarring and the less inflammation the less scarring as well.

You should elevate your head from the horizontal by at least 45 degrees.

Cooling helps too and you should use cold compresses for 15 minutes every hour for at least the first 3 days.

You should avoid hot temperature things like hot liquids, hot showers etc to keep the oozing down to a minimum.

Avoiding blood thinners w weeks before and after surgery is essential to avoid oozing during and after your procedure.

Having your surgeon use the best technique is vital to a great result, and his skill will determine the amount of scarring that will take place.

Along your incisions you should always have the them covered with vaseline, and I would avoid antibacterial ointment to avoid contact dermatitis that is very common after surgery.

Hydrogen peroxide can be used judiciously and with the guidance of your physician but when used without careful regard this agent can increase scarring. Keeping the crusting to a minimum will decrease the bacterial load and lead to less scarring.

After the first 2 weeks, you can then use a silicone based scar gel to cover the incision for the next three months.

External incisions are what you should use your silicone gel for and not the internal incisions inside your nostril.

Minor scar treatments can lead to the best result; a resurfacing 4-8 weeks after your procedure can help!

what to do about columnellar scarring after rhinoplasty from another physician?

This is a question that i answered for a patient who had rhinoplasty done by another surgeon. She had some scarring under the nasal tip in the segment bewteen the nostrils from a worms eye view:

Having your surgeon use the best technique is vital to a great result and his skill will determine the amount of scarring that will take place after Rhinoplasty. Along your incisions, you should always have the them covered with vaseline and I would avoid antibacterial ointment to avoid contact dermatitis that is very common after surgery.

Hydrogen peroxide can be used judiciously and with the guidance of your physician, but when used without careful regard, this agent can increase scarring. Your incisions should look like a very fine wrinkle and this is the goal. When they are fairly clean I would avoid hydrogen peroxide and use vinegar and water in a 1 to 3 dilution or less, depending on your discomfort.

Keeping the crusting to a minimum will decrease the bacterial load and lead to less scarring. After the first 2 weeks, you can then use a silicone based scar gel to cover the incision for the next three months. External incisions are what you should use your silicone gel for and not the internal incisions inside your nostril. Question about the tip being too big and asymmetry after oozing on one side after a rhinoplasty from another surgeon.

Sometimes, when you have more oozing on one side it will be more swollen and cause the nose to look asymmetric which will resolve in a few months. You will continue to change for the first 6 months to a year after a Rhinoplasty. Your doctor will know best what it will likely look like.

Depending on what was done, the tip will likely get more and more refined and look better and better. The breathing can be due to swelling in this early period. Because your tip looks like it goes to the right, it is possible that this is due to a structural issue and that your tip is going to the left which will impinge sometimes the airway on the right side. This may require a revision but it may be too early to tell.

I would discuss this with your doctor and have him look at it. The only way to tell is for me to really look at it in person as well. Don’t worry, all will be well. You can easily revise these things often times.

Judicious use of steroids can help with the asymmetric swelling but it could also lead to its own problems by making the skin really thin and creating abnormalities if the steroids is not injected deeps enough.

Question regarding the tip being too round after rhinoplasty from another surgeon?

This is a questions that I answered for a patient that had a rhinoplasty from another surgeon and felt her nasal tip was too round after the procedure.

If it is above the tip area, this might be because you don’t have a supratip break point.  The supratip area usually should be lower than the actual tip. This helps to distinguish your nasal tip from your nasal bridge and should be above your nasal tip about 1-2mm. Also the angle from your tip as it approaches the columnella (the segment in between your nostrils from a worms eye view) might be a little obtuse and the angularity could be adjusted so that it has more of a contrast as the tip gradually transitions into the columnella, the area below the tip. Four months is still early though and things can change for up to 2 years. You should send me a picture for me to look at it now.

Crooked nose after Rhinoplasty?

This is a question that I answered for someone who had a procedure by another physician.

Crooked nose after Rhinoplasty can sometimes be due to swelling.

Asymmetry with Asian implant rhinoplasty in Seattle is one of the most common complications with this type of surgery.  I believe that it has to do with the approach and the fixation that is used to keep the implant in the middle and not asymmetric. Because it was your second time, there was likely a lot of scarring in your nose from the previous procedure. This can leave scar tissue that can serve as an impediment for a straight result.

Another reason for the asymmetry can be due to inherent asymmetry in the nose. An asymmetric nose will like remain asymmetric unless additional procedures are done to straighten the nose along with the implantation of your nasal implant. The crooked nose is one of the most difficult rhinoplasty cases out there.  Although unconventional, I prefer to do all implants through an open approach.  This allows me to clear scar tissue, open the nose and assure that the plane is correct, ensure that the implant is in the middle and lastly I can fix the implant in the middle with sutures.

Typically I fix the nose to the septum near the bone and septum junction and a point lower than that and then I secure the implant to the lower part of the nose and tip at a total of 4 different locations.  This helps cut down on variables that can lead to asymmetry after rhinoplasty.
