Jaw Silicone Lateral Implants and determining what size you need

In general, we use implantech implants and their silicone jaw implants come in 3 sizes 8,10,12 mm. This is the amount of lateral projection for the jaws. The first way to see how they would look is to place the sizers on the area we put the jaw implants to see how they will look based on the outline that they create. Sizers are the exact same size as the actual implants and are used to test to see how they look in there before opening up the actual implant. These same sizers can be used to determine how they will look on a person. The second option is through morphing and using a computer system that can change your before picture to give you an idea of what it can look like after the procedure.

A word about custom lateral silicone jaw implants. In my opinion, most of the cosmetic concerns that people have can be handled by the implants that are made by implantech. These implants have gone through multiple designs and have been shown to create the most ideal enhancements. In fact, because we do revisions of jaw implants as well. Quite often the custom implants are the ones that seem to require more adjustments or revisions.

Microtia repair in Vietnam by Dr. Philip Young of Bellevue | Seattle.

I was very lucky to be invited to participate in the Medical Mission to Vietnam with the Humantarian Arm of the American Academy of Facial Plastic and Reconstructive Surgery, Face to Face Organization. There we were able to help people who came in with conditions secondary to trauma (a cheek zygomatic maxillary complex fracture and mandible fracture, nasal fracture), congenital issues (microtia, craniofacial deformity), Chemical burns to the face, among other things. In this blog, I will be presenting our case on microtia which is a congenital condition where the ear is arrested in development. Here is a picture of what we worked on:

Essentially, all the lifts such as quick lift, lifestyle lift, s lift, smas lift, deep plane lift, mini lift, traditional face lift, etc are a variation of the basic facelift that we discuss on this page. The facelift entails making an incision in front and behind the ear to pull the face and neck up for facial rejuvenation. The incision in front is mainly for a facelift, while the incision that extends behind the ear is for a neck lift. In general what this explains is that for you face you should first consider the YoungVitalizer which is less invasive. Even before this you can consider fillers with the knowledge that the fillers last 6 months to a year at most.  After doing less invasive procedures you can move on to more traditional procedures if needed. If jowls are your main issue a facelift would be ideal. If the neck is your concern, a necklift would be the best option.

What the page will go into is also the idea is bringing more light back to the face. We talk about the aging process on this page and how the changing of the grape to a raisin makes the tissues draw or move inward and downward. This movement effectively causes a shadow to be cast over the face by the structures above them.  For example the brows start to cover the upper eyelid. For the lower eyelids, the eyes and brows start to cover the lower eyelids and cheeks.  For the nasolabial folds, it is the cheeks that start to cover the folds and make them look deeper. For the marionette lines, the lower cheek and mouth help to create more shadowing in the marionette area, etc, etc.

Jaw Augmentation Question and Answer Session by Dr. Philip Young of Bellevue & Aesthetic Facial Plastic Surgery:

Here are a few questions that someone wrote me regarding Jaw Augmentation that someone recently asked us:

1) What are the associated risk with this kind of surgery? i read online that these kind of procedure has a higher degree of infection rate when compare to other implant procedure. my concern is that since i am not from the states, so would have to travel to all the way from Asia for this surgery. if infection does occur, i would have to travel all the way back for to have the implant removed, a risk i am not too keen of. what are the infection rate of those who have under go mandible implant at your clinic?

The rate of infection is very low but lateral jaw implants are more likely to occur than other implants. They can get infected but that usually occurs the first week and we know how to take care of infections and let you keep your implant. This is in contrast to other surgeons who do implants, with infections they usually take them out. We know how to treat the infection with the implants in place so that you can keep your implants and not have to undergo another surgery to replace them.

 

2) if infection doesn’t occur in the first 3 to 4 weeks, what are the chances that infection might occur several months or several years later? i heard that infection might still occur even if infection doesn’t occur initially. what is your take on this?

After the first week it is much rarer and you would really have to traumatize the area to get it infected or something. There would have to be a blood collection of some sort and a violiation of the sterility that is there for an infection to breakout. If they do get infected later, we usually treat this situation like if it were infected early on after the surgical procedure.

 

3) once the implant has been inserted, does it cause any restriction in term of mouth movement or chewing or talking? and does the silicone implant grind at the mandible bone over the years? since the silicone is screw to the mandible, there is a chance that the the through wear and tear, the silicone might wear down the mandible bone.

It will restrict your movement for a few months at the beginning but this hasn’t been a major issue. The mandible is very strong and the implants are placed in this strong area of bone. The chance of erosion is very small and if it were to occur, the amount of erosion is small and unnoticeable as well. The silicone implants tend to erode the soft bone. We have seen this in chin implants when they are placed higher and not at the bottom of the chin bone. The higher location starts to involve the alveolar soft bone around the teeth.

 

4) do you custom design or custom make the implant base on individual patient requirement? beside implant, what that the option? do you use bone paste or bone cement as well?

Implants work the best and you can do custom implants but they are much more expensive. Bone paste and cement do not keep there shape as well as implants. But they can give you some volume temporarily. We do custom implants when a person needs both chin and jaw implants sometimes. The cost of these custom implants are somewhat prohibitive. Usually it costs several hundred dollars for the ct scan and the process with the company can be 2-3 thousand and this is just for the implant. With the surgery on top of that it can put you in the 10k range.

We wanted to discuss steroid injections to the nose and introduce our video on this subject with a live demonstration of the steroid injections. We also dilute the kenalog to make it so the results are more subtle, slower, conservative and with less complications. Steroid injections to the nose are best used if there are localized areas of swelling and not general swelling. If it is generally swollen, we like to give oral steroids like a medrol dose pack. You can go on to rxlist.com to read about the risks and benefits for your own knowledge.  Some people are afraid of steroid injections. The complication of steroid injections include that the skin can get thinned out and there can be dents that form if you dont do the steroid injections correct and in the wrong plane. The key to doing the injections is getting it in the right plane which we have a lot of experience doing. Also we stay away from strong dosing of steroid injections. We like to do low dose steroids, diluted and more frequent. This cuts down on a lot of the complications that you can get with steroid injections.

Other people are afraid of the pain of steroid injections in the nose for after a rhinoplasty procedure. We cut down on the discomfort by using topical anesthesia and then using some local anesthesia in the steroid injections themselves to further increase the comfort.  This is all demonstrated in our video. What are some surgeries that you can do to make your face change into a different sex. One of the main procedures that Dr. Young believes will make a big impact is a Rhinoplasty. Asian Ethnic Laser Resurfacing by Dr Philip Young Aesthetic Facial Plastic Surgery Bellevue | Seattle: Asian patients tend to develop moles and sunspots more so than the rest of the patient population. This is also something that is found other ethnicities. We explain some topics on hyperpigmentation and hypopigmentation on our Asian Ethnic Laser Resurfacing Page. Hyperpigmentation can happen as you age.  As you can see in the before and afters above, the laser resurfacing really helped this person out. Some of the moles are still there and these can be addressed as well. This particular patient wanted overall improvement and didn’t want all of the moles gone on his face. Laser resurfacing can be used to take the top layers of the skin down so that the deeper skin cells are able to resurface | reline the skin that has been taken away. In this process, the sun spots are reduced and fine wrinkles are smoothed away and improved. You have to be really careful to keep the laser resurfacing at a safe plane or depth.
