But these solutions are more temporary and last anywhere from 6 months (Restylane) to a year or more (Radiesse). Fat injections are another solution to augment the nose to hide the bump (or nasal hump / nasal bump / hook nose / prominent nose / large nasal bridge / nasal bridge hump).  When I talk about these injections I mean injecting the fillers above and below the bump to hide it better. This type of procedure can be done in the office under minor local anesthesia with minimal bruising and swelling that lasts only a couple of days versus a week or more for rhinoplasty.

ARhinoplasty (or nose shaping / nose reshaping / nose plastic surgery / nose cosmetic surgery) is essentially any procedure that alters the shape of your nose and that can mean what I just described to the more traditional thought of during a more invasive surgical procedure to change the shape of the nose. A rhinoplasty has been called a nose job or referred to as nose reshaping / shaping, etc.  From a more traditional standpoint, taking down a hump will entail making small incisions within the nose to file down the bump.  Sometimes in order to make the nose appear more normal, you need to cause controlled breaks in the nasal bones to make it close in after the filing takes down the hump.  This type of approach is more longer lasting but does take a little while longer to heal versus the filling techniques that I described above.

What do you do for lower eyelid bags, dark circles for someone in their lates 30’s early 40’s?

Anullcan (or lower eyelid lift / lower eyelid blepharoplasty / lower eyelift / lower eyelid pinch) help by taking away extra skin and removing fat in the lower eyelid (this can also help lower eyelid bags, dark circles, puffy eye bags, dark under eyes). What are the options for sunken cheeks besides temporary dermal fillers?

This is a questions I answered for someone that was wondering what option there were for sunken cheeks besides temporary Facial FillersFat Transfer or Facial Implants can give you the long term volume you desire in the cheek.

Sunken cheeks is a natural aging process.  When you age you lose fat in your cheeks and this can lead to your tissues descending, wrinkles developing, folds developing, lower eyelid fat protruding, eyelid bags, etc.  Dermal fillers like restylane, juvederm, radiesse, perlane, are temporary and last from 6 months to a year or more.  Of these fillers radiesse is a bone product made up of calcium hydroxyapatite while the others are made up of hyuloronic acid.  There are more permanent fillers like silicone injections, and artefill.  The problem with permanent fillers is that they have the potential to cause infections, scarring and granulomas (balls of chronic infection) that can present years later after you think everything is okay.

Other more permanent options for sunken cheeks include fat injections and silicone implants.  Silicone is very compatible to place in the cheeks.  The silicone atom is one step below carbon on the atomic chart and hence makes it really compatible.  The infection rate with silicone is very low and its use has a long history of safety.  Silicone implants can be placed within the cheek to give a person a more permanent volumizing and lift up sunken cheeks.

Fat injections are another way of volumizing the cheek. Perrmanency depends on whether the fat survives.  This depends on many factors: technique, health of the patient, care of the patient after the procedure, etc.  If fat lasts for more than 6 months to a year, it will likely survive for many years sometimes 10-15 years or longer.

Can a forehead augmentation be done after a brow lift?

Forehead augmentation can be done but can affect the results of previous browlifts. You can augment the forehead with silicone after a browlift.  One of the issues is that the augmentation will affect what was done with the previous browlift and you will sometimes need to refix the brows and allow them to fixate in the newer position.  Browlifts depend on the fixation of the bone to the bone covering and the soft tissue that covers the forehead.  When you dissect to implant something into the forehead it will break that bond.  Also with the implant fixating the brows to a new position will present some challenges as well depending on where the implant is placed.  Implants placed in the forehead can establish needed volume that is lossed with aging.  Most of the time, however, forehead augmentation is used to fill in defects from trauma and for reconstructive purposes. Also depending on what implant is used and the size you may need longer incisions such as a variation of the coronal incision that goes from ear to ear.  Not all of this incision is needed most of the time.  In terms of augmentation, you can use silicone sheets / blocks, Facial Fillers or fat augmentation.  Silicone and fat are the options that offer more of a longer lasting results with silicone being more permanent than fat.  Fat injections, however, can be done with no incisions most of the time with less downtime associated with long incisions. Augmenting the forehead can potentially help with wrinkles in the forehead, eyebrow drooping, extra eyelid skin and temple hollowing depending on how and where the augmentation is done.

Non Surgical Face Lift

There are many questions out there regarding an alternative for a non surgical facelift.  There are ways to tighten up your face without making incisions.  One way to do this is to undergo a laser resurfacing. We have the lumenis Laser Resurfacing unit and this is proven to be the best one out there on the market. It is the safest and most versatile of all co2 laser units and this can improve your skin by decreasing the wrinkles, improving skin texture, decreasing pore size, tightening the skin and creating collagen under the skin surface to create some more youthful volume.  Co2 laser resurfacing can tighten your neck skin and laxity as well. Dark Circles, Jowls and whether a facelift of some sort can help

In terms of a null, you might be a little early to have one.  It would really be beneficial to see a picture of you without smiling. A facelift can lift your jowls and improve your nasolabial folds to some degree. it won’t help your dark circles, and bags under your eyes.  Most surgeons would advocate a lower eyelid blepharoplasty, but this could cause hollowing under your eyes and appear unnatural.  I would suggest filling your cheek and lower eyelid with some type of Facial Fillers.  With filler you could go with temporary fillers or try the YoungLift.  Fillers such as Restylane, Juvederm, Radiesse, Perlane, can all fill in the cheeks and partially the lower eyelid but are temporary and last 6months to a year or more.  Permanent fillers such as silicone and artefill are risky and I stay away from them.  Fat injections can help and if the fat survives for more than 6 months to a year it will stay there for years sometimes more than 10 years.

Radiesse injections to the nasal bridge and how long to wait before rhinoplasty

If the augmentation is lost, I think it would be okay to consider Rhinoplasty. Facial Fillersis made up of 30% hydroxyapatite and 70% of the carrier. Much of the carrier dissipates over the course of the year.  It might be prudent to wait. But if you think the results are essentially back to normal you could consider rhinoplasty and an implant to raise your nasal bridge. What is left in your nose is likely the hydroxyapatite and this could stay in your nose for years. Biopsies of the bladder have shown it to be there for 5 years.  Hence if most of the augmentation is gone you could do the rhinoplasty earlier than the full year that it is expected to give you results.  If you really wanted to be safe you could wait for the full year to pass.

Altering the shape of the philtrum / the center portion of the upper lip.

Definitely photos are always beneficial to help a surgeon decide what would be the best solution for a particular problem and to reach the desires of the patient.  You can reduce the size of the philtrum by excising certain portions of the lip to change the appearance through a directed approach to Lip Augmentation.  These excisions can occur inside the mouth and can hide quite well.  Sometimes making incisions under your nose can also help change the shape of the lips to get what you want.   There are also many different lip advancements that can be done to alter the shape of the lips.  Injections can be helpful but would really depend what the desired outcome was.  I’m sure if you brought your picture in that a facil plastic surgeon would be able to figure out what was best!

Best treatment for tear trough: lasers, fillers, or fat injections?

The tear trough is a tough area to treat through eye lifts / blepharoplasty procedures. This area is located between the nose and the lower eyelid and is situated not above the bone as most people believe but above the soft tissue of the eye socket and eyelid. People have tried repositioning of the eye socket fat into that area through a blepharoplasty / eyelift procedure.  This can work but has been shown to lead to eye position abnormalities and problems with dry eyes, etc. Fat Transfercan work in this area and may be the most natural way to rejuvenate this area.  It does have some issues related to survival of the fat.  But is the fat survives past a year it will likely stay in the area for many years, sometimes 10-15 years.   Fat grafting requires a lot of skill and knowledge of fat survival techniques.  Facial Fillers can fill up the volume loss that has lead to the tear trough appearance and I’ve done that many times and have a lot of patients that get this done.  The key is being able to fill up the area without leading to bumps and ridges.  This requires some skill and with a surgeon’s experience it can be done to look very nice and satisy your clients.  Fillers can look not as natural as fat, it can lead to a translucent type of appearance and some dark appearance of the filler within the skin.  But, in general >95% or more patients are very, very happy that they had filler placed to treat the tear trough area. Laser Resurfacing can create a layer of collagen underneath the eyelid skin to camouflage the dark appearance of the muscle underneath the skin.  It can also tighten the skin and reduce the unwanted wrinkles in the lower eyelid.   However, lasers will not replace the volume that is lossed with aging around the lower eyelid area.  This is where fillers and fat injections come into play.

What is the best method of injecting radiesse, nerve blocks, ice, etc?

There are many methods of injecting radiesse so that it is not as painful.  Nerve blocks are the best way to make parts of the face numb. You can numb almost the whole face by injecting a few small areas that affect large nerves that control the sensation to large areas.  Local anesthesia can then augment the numbness.  You can also add lidocaine into the Radiesse.  2% lidocaine is the best to use with the radiesse.  Topicals can also help out to insure that the area will not have any pain during the injection.  This can help when the regional blocks do not make the complete area numb.  Ice is another way to help make the injections less painful.  They are also helpful when you do the initial regional blocks to lessen the needle pain when these regional blocks are done.  Radiesse injections can be painful without anesthesia so all of these techniques can augment this experience. A dental block may not be the only thing you need.  There are other blocks you can do to the face to make almost the whole face numb.
