What percentage of body fat do you need to be a candidate for fat grafting?

Studies looking into the percentage of body fat are not prevalent in the literature. So this hasn’t been determined accurately. Because you work out regularly and have low body fat, you might not be the best candidate for a lot of Fat Transfer(fat injections, fat transfer, fat filler, fat injectible). I would reserve fat grafting for areas that are least likely to tolerate more permanent fillers or implants.  These areas would be around the eyes and in the cheek hollow area.  The alternative for the areas other than that are sculptra, silicone implants for a more semi permanent or permanent options.  Temporary fillers can be used everywhere essentially.  Temple hollows, forehead volume loss, jaw shrinkage and sunken cheeks can be augmented with silicone implants as an option in the very thin or fit patient.  Fat can then be used in the areas that cannot tolerate these implants.

Should I do a chemical peel under the eyes or a filler first.

Aging is in a large part due to a volume loss within the face.  As we age we lose fat within our face, our bones get smaller and our skin thickness goes down too. A simple way to look at it is that we change slowly from a grape to a raisin.  Resurfacing in my opinion should always be done after you volumize or reshape a certain area.  This is more applicable to more permanent or semi permanent filling and reshaping procedures such as face lifting and fat grafting to the face.  But I generally follow that same principle even with Facial Fillers.   Many times you will find that you will need to do less resurfacing after these more permanent procedures and semi permanent procedures. Also after these reshaping procedures, the wrinkles are less prominent and also respond better to resurfacing procedures like chemical peels and laser resurfacing because more of the surface becomes exposed.

My right eyebrow is higher than my left. Is there a subtle way to correct this?

This is a question I recently answered for someone:

Asymmetry in the eye area is normal.  In fact, we appreciate beauty in an asymmetric way.  Our right brains appreciate beauty more than our left and our visual fields are asymmetric.  Essentialy, in short, we see someone’s right face more when we regard or assess beauty.  This has evolutionarily lead to asymmetries in our face based on millions of years of sexual selection.  We tend to choose our mates based on how good the right sides of our face look more than what our left sides look.  Thats sort of the long story and there is even a longer explanation to that.

In terms of correcting, you should realize that not everybody is the same on both sides.  This may comfort you a little and might dissuade you from getting something done.  What I can say is that you can get some correction of this if you are willing to do this. From this picture, I would really have to see you in person or see pictures of you to really know, but your left eyebrow is a little lower.  You might benefit from adding volume to the left temple region and above the eyebrow to lift the eyebrow subtly.  Even adding some volume below the left eyebrow could benefit.  This can be done with Fat Transfer, Facial Fillers, and other implants.  A browlift could even pull up the left eyebrow but in a less than natural way.

But I would really have to see you in person to be able to counsel you more.

Fillers, Botox or Lasers for the Under Lower Eyelid Area?

This was a question I answered for someone recently:

Restylane is a filler and can be used to fill in the lost volume that is really the cause of the majority of the reason why your lower eyelid looks the way it does. Fat injections, the YoungLift, Juvederm, Perlane, Radiesse are other fillers that can be filled into the lower eyelid area.  The temporary fillers last from 6 months (restylane) to a year or more (radiesse) with perlane and juvederm in between.  Fat injections can last much longer but this isn’t guaranteed but is the best shot for a long term correction.

Botox prevents the muscle around the eyes from squinting which can make the wrinkles less noticeable. Dysport (or botox / bo tox / botulinum / neurotoxin / disport) is a great option.  But these do nothing for the surface quality of the skin and neither does the filler. But the fillers can decrease the amount of wrinkles by filling up the volume.

This is where the co2 laser or other resurfacing procedure (erbium yag, active fx, ultrapulse, deep fx, max fx, total fx, traditional co2 laser resurfacing, laser peel) can help with by decreasing the wrinkles and actually tightening the skin.  A small amount of volumizing also occurs with lasers with their creating of a thin collagen layer deep to the skin.

What is the best to replace volume in the upper cheeks filler or fat injections?

Both are good options with positives and negatives. Facial Fillersused for the upper cheeks include radiesse, restylane, juvederm, perlane. These are temporary. Radiesse should only be used in the deeper planes of the cheek especially around the lower eyelids.  The white appearance of radiesse can be seen through the thin skin of the eyelids if you are not careful and place it too superficial. The others are hyaluronic acids and they are clear looking and can be place almost anywhere.  Because they are clear when they are injected into areas of thin skin, your skin can take on a translucent look and sometimes this can look like a very light bruise.  They are fairly easy to put in and you can get relatively quick results but they are temporary lasting anywhere from 6 months to a year or more. Restylane lasts about 6-9mo, juvederm maybe a little longer but it comes supplied with less material 8cc compared to 1.1cc of restylane. Radiesse lasts about a year sometimes more or less.

Fat injections however can last for many years.  Usually you can get approximately 60-90% of the fat to survive depending on how well you do the procedure. Because it is a living tissue technique is vital to insure survival of the fat.  When carefully placed fat is the most naturally looking of all filling or volumizing type of procedures.

Can radiesse / radiance be used in the Jawline and lateral portion of the Jaw area?

Radiesse can work but you may need a bit more in this area. Radiesse is made of calcium hydroxyapatite, the same constituent as your own bone.  This makes it really compatible with our bodies. For Mandibular augmentation, you will likely need a siginificant more amount of augmentation here than you would along the mouth lines.  Mandibular silicone implants are made up of approximately 10-15cc each side. So you will need quite alot of radiesse to show the same amount of change.  This is important to know before doing radiesse injections in this area.  If you just want to get a little taste of what this will look like then radiesse would be a good trial.  Silicone implants are one of the better ways of really augmenting the lateral jaw bone.

What are some options for filling in dents in the cheek?

Did Megan Fox have juvederm of collagen? What does she use?

You can’t really tell from this photograph. Even without makeup it is hard to tell if someone had juvederm or another Facial Fillersand collagen.  The difference would be that with one of the hyaluronic acids a translucent type of look is more likely than with collagen.  But in the lips it is hard to tell.  Most people can’t tell the difference.  Here with Megan Fox’s picture her lipstick makes it even harder.  One thing with lip augmentation is that you shouldn’t make the upper lips the same size as the lower lips which is often done in Hollywood. This wrecks the balance of the lips.  You can read more about why that is from my theory on facial beauty called the Circles of Prominence on my website. In general, hyaluronic acids have taken over collagen as a filler because you don’t have to have a skin test, the chances for a reaction are much lower, and the results last longer with hyaluronic acids like restylane, perlane, juverderm.

Can botox and fillers make you look worse after they wear off?

People tend  to forget about how they used to look like after they get their improvement from Botox (or dysport / botulinum toxin / bo tox) and Facial Fillers (such as juvederm / restylane / radiesse / perlane / cosmoderm / artefill / sculptra / alloderm).  This is a very common occurence.  Hence when the effects wear off, people tend to think they look worse when in fact they are just returning to a previous less optimal state. In fact, botox will only help make the muscles weaker the more you do it which will essentially leave the muscle less likely to make more wrinkles. Fillers also have a possibility to make things better even after the effects are long expected to be gone.  Sometimes, fillers develop scar tissue around it which makes the fillers last longer and hence the beneficial effects last longer.  The scar tissue prevents the body from breaking it down. This is something that I have noticed in many patients.  But, back to the question, i don’t think your appearance is usually made to look worse after these temporary treatments. In general, botox can improve the appearance of dynamic wrinkles (wrinkle treatment / facial line treatment) while fillers can improve more static wrinkles by filling in loss volume.

Eyelifts / Browlifts / Fillers and how to make the eye look younger again.

There are many procedures out there that can be done to improve the way the eye looks and to reverse some of the signs of aging (including droopy eyes, extra eyelid skin, tired looking eyes, puffy eyes, eye bags, eye wrinkles).  An eyelift / blepharoplasty (or eye lift / eye plastic surgery) procedure is one way and this entails removing skin from the upper eyelid. Sometimes this entails removing skin, muscle, fat and also recreating the eyelid crease.  Recreating the eyelid crease is perhaps the one thing that can really make an eye look better by reconstituting this crease which can really open up the eye and it appearance.  This is a really complicated part of an eye lift and requires understanding the anatomy to a greater degree and an increase level of expertise.  I have a lot of experience recreating the crease and I have done this with my Asian clientele but I also apply it to my caucasian clientele and other ethnicities.

Browlifts (or brow lift / forehead lift / brow plasty / browplasty / eye brow lift / eyebrow lift) are another way to open up the eye by pulling up the eyebrow to a more youthful position (other signs of aging include droopy eyebrows, hooded eye brows, extra eyelid skin).  This is done through incisions that are placed behind the hairline and hide really well.  Sometimes an incision in the eyelid crease is necessary to better expose the orbital bone above the eyelid and to allow a better approach to lift the eyebrow.  It is important for you to go to a surgeon that understandsFacial Beautyto get those excellent results.  You may have seen people who have a surprised look after a brow lift.  This is direct by product of our limited understanding of facial beuaty. While Radiesse, a product of bone made up of calcium hydroxyapatite, can last up to year or more.

Both fat and fillers can be placed around the eye to really bring youth to the appearance of the eye.  But a knowledge of facial beauty is much more than half the battle in rejuvenating the eye.
