How long does radiesse last?

Saturday, June 27th, 2009

Facial Fillers slowly goes down with time but can last for longer than a year. In fact biopsies of areas that have had radiesse injections have shown that it can last up to 3 years or more.  But the volume that remains later on is no more than 30% of the original volume that was injected. The area that is injected also determines how long it lasts.  In more mobile areas, the effect is less and goes away faster.

Another way of filling is through fat injections.

I refined an amazing volumizing procedure called “the YoungLift”.  This is an innovative technique that volumizes your face to bring out the younger and natural you without looking like you had something done.  This is not a facelift and requires no incisions.  Only pinpoint puncture sites are used and these heal imperceptibly.  The YoungLift can be done without general anesthesia and without drains, and large bandages.  You also have a lot less downtime and discomfort compared with traditional facelift procedures.  Dr Young employs the very best techniques from around the world into one volumizing procedure and he uses his internationally acclaimed understanding of facial beauty to create the youthful volume you once had.

Fat grafting and hard feeling of the grafted fat

Saturday, June 27th, 2009

This is in regards to a person having fat grafting by another surgeon and questions regarding the fat feeling hard and whether it is normal.

Fat Transfer is an excellent way to bring youth to your face. Losing volume plays a huge part in the aging process. Hence, replacing this fat to the face can reverse a lot of the aging that has occurred within the face.

One rule in fat grafting is that fat will take on the characteristics of the tissue that it is placed near or on. Hence the fat that is placed on your cheek bone will feel like bone, etc. During the post procedure phase, however, the fat will feel hard due to the inflammation that it is undergoing. Fat is one of the most inflammatory substances in the body and hence it will lead to a lot of inflammation.

The key during the healing phase is to be patient. It will eventually soften and feel more like your own tissue.

Consulting a Board Certified Facial Plastic Surgeon would be something that I recommend. We specialize in the face and are highly qualified to help you with this.

Platelet rich plasma, centrifuge topics in fat injections

Saturday, June 27th, 2009

I think that you doctor is right to some degree regarding platelet rich plasma and centrifuging the fat. Fat can be something that can improve scars. Although I think depressed scars are a little harder to correct with fat grafting. Fillers were suggested by another doctor but you better be ready to continue to pay for additional filler every year or less to keep the condition corrected.

Platelet rich plasma will only introduce more inflammation to your Fat Transfer and will ultimately lead to less survival in my opinion and in agreement with your doctor. Centrifuging can lead to more fat cell death, but you have to isolate the viable fat cells some way. If you don’t centrifuge the fat, you will have a lot of inflammatory components in the harvest that will affect your fat’s survival. So, you do need to somehow isolate the cells that are most likely to survive.

Waiting for the fat to settle is another approach but the time it takes to do this can be a detriment as well because the longer the fat stays out of the body, the more the fat will not survive.

I believe that centrifuging at a slow rate is probably the best median between the two. This has also been proven in studies on centrifuging fat and fat’s ultimate survival. The usual standard is at an rpm of 3000 for 5 min. This has proven to be too fast and can affect the fat cell’s survival. I typically centrifuge at the lowest speeds and do it for 3 mins max.

tear trough lump after fat injections / fat transfer

Saturday, June 27th, 2009

There are things you can do to improve it. Waiting is one option. But you have to tailor the option to the desire of the patient. One option is to inject steroids, but you have to be aware of the risks of this.

The next option is to actually make an incision to take out the Fat Transfer. Although this will leave a scar, but you can always improve the appearance of the scar. Another option is to do micro liposuction to remove the lump, and this takes experience. Try to find a surgeon willing to help and then you have to be patient because it could take a number of procedures to make it just right. The body is complex and there are many variables that can affect the outcome of your procedure.

Another option is to add fat in the right areas to make it so that the lump is no longer as noticeable.

Fat injections to the cheeks and nose done by another surgeon 3 days ago and questions on swelling

Saturday, June 27th, 2009

Three days is way too early to tell what you are going to look like.Fat Transfer, depending on the technique that you use, creates a lot of manipulation to the tissues. This is a consequence of having to layer the fat into multiple separate layers in order to help the fat survive. This manipulation can create a lot of inflammation after your procedure and this can lead to the swelling that you are experiencing right now.

Sometimes, this swelling and bruising can take several weeks to go away. I would definitely not do anything this soon. This could be a big mistake. If you do anything now you can have other effects that you weren’t intending to have. You should definitely bring these concerns to your doctor. Your doctor will help you through this. There are a lot of amazing doctors out there and your doctor, I’m sure, is one of them. Just make him aware that you are concerned.

When all of the swelling goes down, you will then be able to tell if you need anything else done. Don’t worry, there are always options to help your situation. You can always add more fat to make the fat that is causing your concerns blend into the surrounding area. You can always remove fat through an incision, or through micro liposuction. Steroid injections can also be an alternative to consider. One thing you could consider is oral steroids at this point in your recovery. This could help your swelling. But make sure that you know of the potential risks involved with steroid use.

Placing pressure in the lower eyelids after fat injections and the effect on survival?

Saturday, June 27th, 2009

First of all, I want to tell you that I’m sorry that you felt mistreated. Sometimes, doctors are very busy and they might come across that way when really they have a lot on their mind. A good bedside manner really can make a difference though.

This was a question I answered for someone who had a Fat Transfer procedure to her lower eyelid and wondering about her doctor applying pressure to her lower eyelid.  This was a procedure that was done by another doctor.

In terms of the pressure, applying this judiciously can help some swelling that may be related to the manipulation caused by the procedure. Another word for this is inflammatory edema. It shouldn’t really affect the way the fat will be shaped unless a lot of pressure is applied in a forceful way that could affect the fat’s survival during the crucial period of about a month after your procedure.

The fat placed really should be formed by the technique of laying the fat into layers and by the amount placed. Each fat cell or group of cells should have their own specific location to acquire their own blood supply. This is the best way to ensure that the fat will survive.

Fat injections in the temple to elevate the eyebrows?

Saturday, June 27th, 2009

Fat Transfer are the most complicated procedure that a plastic surgeon can do in regards to rejuvenating the face. When fat is placed in the temple and lateral brow area, a modest amount of elevation of the brow can occur. The key is grafting the fat so that it survives to keep the volume and the lift that it has in the lateral brow area.

Oftentimes, a brow lift will be something that can’t be avoided. In studying the brow area, I have found that when fat grafting is concentrated in certain areas, a lateral brow lift can occur in a very beautiful way. The key to achieving this is very elusive. I have some unique insight into this that has not been previously been known. My plan is to publish this in a future article. In short, this can be done, but this area of plastic surgeon is a fresh frontier that I think is being elucidated as we speak!

Dr Young employs the very best techniques from around the world into one volumizing procedure and he uses his internationally acclaimed understanding of facial beauty to create the youthful volume you once had.

fat grafting or restylane for under eye hollows?

Saturday, June 27th, 2009

Facial Fillers will last about 6-9 months but can improve this area. Fat injections can last 8-10 or more years. You are one the right track though. You need volume in this area and that’s the best way to improve this area. One thing to realize is that restylane will go away and can also be taken out earlier if you wish through enzyme injections. This can be reassuring.

Fat injections are more permanent and is a little harder to get rid of if you don’t like the volume but can be done through different special procedures. In my opinion, fat injections / transfer leads to the most natural results.

Fat grafting or facial implants?

Saturday, June 27th, 2009

Some people are afraid of having Facial Implants and this an important thing to consider. I usually use implants to volumize the face when a person does not have a lot of fat to use, for example in women that are physically fit and have little body fat or if they don’t want to go through the harvesting of the fat that requires liposuction in another area of the body.

After implantation, you can then use the little fat that the person has to augment the implant. I usually do this in two phases with the implant first and then fat grafting at a later date. You also have to consider that fat and implants have different characteristics as well.

Implants require more incisions and usually are placed through the mouth. Fat transfer requires small holes instead of long incisions. Implants can also help in that fat transfering is needed less and cuts down on the need for more extensive fat harvesting.

Deep set eyes and too much lid showing after blepharoplasty by another surgeon

Saturday, June 27th, 2009

The deep set eyes are a part of the aging process. You can do fat grafting to improve this. The larger eyelid showing after your blepharoplasty might improve over time.  As your eyelids rest and the swelling goes down the eyelids will show less. Fat grafting is comlicated so you want to go to someone that has a lot of experience with fat grafting around the eyes. My opinion is that volumizing is really important to bring back the youthful volume that people used to have when they were younger.
