Can a forehead augmentation be done after a brow lift?

Forehead augmentation can be done but can affect the results of previous browlifts. You can augment the forehead with silicone after a browlift.  One of the issues is that the augmentation will affect what was done with the previous browlift and you will sometimes need to refix the brows and allow them to fixate in the newer position.  Browlifts depend on the fixation of the bone to the bone covering and the soft tissue that covers the forehead.  When you dissect to implant something into the forehead it will break that bond.  Also with the implant fixating the brows to a new position will present some challenges as well depending on where the implant is placed.  Implants placed in the forehead can establish needed volume that is lossed with aging.  Most of the time, however, forehead augmentation is used to fill in defects from trauma and for reconstructive purposes. Also depending on what implant is used and the size you may need longer incisions such as a variation of the coronal incision that goes from ear to ear.  Not all of this incision is needed most of the time.  In terms of augmentation, you can use silicone sheets / blocks, Facial Fillers or fat augmentation.  Silicone and fat are the options that offer more of a longer lasting results with silicone being more permanent than fat.  Fat injections, however, can be done with no incisions most of the time with less downtime associated with long incisions. Non Surgical Face Lift

There are many questions out there regarding an alternative for a non surgical facelift.  There are ways to tighten up your face without making incisions.  One way to do this is to undergo a laser resurfacing. Dark Circles, Jowls and whether a facelift of some sort can help

In terms of a null, you might be a little early to have one.  It would really be beneficial to see a picture of you without smiling. A facelift can lift your jowls and improve your nasolabial folds to some degree. it won’t help your dark circles, and bags under your eyes.  Most surgeons would advocate a lower eyelid blepharoplasty, but this could cause hollowing under your eyes and appear unnatural.  I would suggest filling your cheek and lower eyelid with some type of Facial Fillers.  With filler you could go with temporary fillers or try the YoungLift.  Fillers such as Restylane, Juvederm, Radiesse, Perlane, can all fill in the cheeks and partially the lower eyelid but are temporary and last 6months to a year or more.  Permanent fillers such as silicone and artefill are risky and I stay away from them.  Fat injections can help and if the fat survives for more than 6 months to a year it will stay there for years sometimes more than 10 years.

Fat injections in the neck?

Fat Transferin the neck can work if done right.  The key would be to have a result where the fat survives after being transferred into the neck.  Once the fat survives past a year, it should stay in the area that it was implanted for many years.  Some surgeons have been able to maintain augmentation from fat from 10-15 years.  Lumpiness can be a problem but if the surgeon respects how to place the fat this becomes less of an issue.  When I graft fat into the face and neck, whenever I’m superficial I always use finer cannulas to transfer the fat.  I also lay the fat down as evenly as possible so that each fat transfer aliquot is placed as close as possible to the ones placed next to it.  When the fat is injected into the deeper layers, you have more flexibility and less margin for error when you use larger cannulas that deliver larger fat parcels.  Fat injections in the neck may not be perfect but one can strive to perfect this.  There may not be a better way to volumize the neck.  Ultimately, I think surgeons and scientists are going to figure out how to make fat survive better and this will be the main way that surgeons will volumize the face.

Best treatment for tear trough: lasers, fillers, or fat injections?

The tear trough is a tough area to treat through eye lifts / blepharoplasty procedures. This area is located between the nose and the lower eyelid and is situated not above the bone as most people believe but above the soft tissue of the eye socket and eyelid. People have tried repositioning of the eye socket fat into that area through a blepharoplasty / eyelift procedure.  This can work but has been shown to lead to eye position abnormalities and problems with dry eyes, etc. Fat Transfercan work in this area and may be the most natural way to rejuvenate this area.  It does have some issues related to survival of the fat.  But is the fat survives past a year it will likely stay in the area for many years, sometimes 10-15 years.   Fat grafting requires a lot of skill and knowledge of fat survival techniques.  Facial Fillers can fill up the volume loss that has lead to the tear trough appearance and I’ve done that many times and have a lot of patients that get this done.  The key is being able to fill up the area without leading to bumps and ridges.  This requires some skill and with a surgeon’s experience it can be done to look very nice and satisy your clients.  Fillers can look not as natural as fat, it can lead to a translucent type of appearance and some dark appearance of the filler within the skin.  But, in general >95% or more patients are very, very happy that they had filler placed to treat the tear trough area. Laser Resurfacing can create a layer of collagen underneath the eyelid skin to camouflage the dark appearance of the muscle underneath the skin.  It can also tighten the skin and reduce the unwanted wrinkles in the lower eyelid.   However, lasers will not replace the volume that is lossed with aging around the lower eyelid area.  This is where fillers and fat injections come into play.

How do you build up the cheek?

Building up the cheek can be done through multiple different means. This can be done with various fillers including restylane, juverderm, more radiesse, fat injections, or facial implants.  Either of these options have benefits and drawbacks.  The fillers are temporary and you will need reaugmentation in 6 months to a year.  Facial implants will last essentially forever but need to be done in the exact way and augment your cheek in the best way.  When you place Cheek Plastic Surgery into the cheek area, sometimes the cheek can be fixed on the cheek bone with metal screws and if there is less than ideal augmentation more or less then you can tailor the augmentation through different means.  If you have too much augmentation, you can tailor the cheek implant with burrs if the cheek implant is solidly fixed with the screw.  If it is not big enough you can always place a bigger implant or  fix more silicone onto the existing implant with sutures. The key with silicone or other implants is to have the implant fixed so its stable, not moving and not collecting fluid between it and the bone.  These are things that can prevent infection and rejection. Silicone is probably the safest and most stable and non reactive implant that you can place.  Other implants create more inflammation and have a higher incidence of rejection and infection.

Fat injections are another way of building up the cheek.  The survival of fat is variable but if the fat survives after the first 6 months to a year the fat will likely survive for years.

Restylane or Arcus Marginalis release for lower eyelid dark circles

Both of those options can help. I think Restylane would be a better alternative to replace the volume that you have lost in the lower eyelid and cheek area. That is essentially what has happened — a loss of volume. The only thing is that restylane will last only 6-9 months. Fat repositioning can help the condition but there are limitations. It can leave you looking hollow and it can impact the position of your eyelids and can have complications.

I don’t prefer the fat repositioning procedure. I think volumizing the area is the best way to rejuvenate the face and the lower eyelid area. It can give you the most naturally looking results.  Fat transfer to this area is the best if done right.

Restylane for the dark circles under the eyes

Restylane will last about 6-9 months but can improve this area. Fat injections can last 8-10 or more years if the fat survives after the first year. You are on the right track though. You need volume in this area and that’s the best way to improve this area. One thing to realize is that restylane will go away and can also be taken out earlier if you wish through enzyme injections. This can be reassuring.

Fat injections are more permanent and is a little harder to get rid of if you don’t like the volume but can be done through different special procedures. In my opinion, fat injections / transfer leads to the most natural results.

What to do about the neck bands a questions I answered for a patient who had things done by another doctor

You didn’t mention what was most bothersome but I’m assuming its the banding in your neck. Botox can help improve this banding and will last 3-6 months. Another way of doing this is by doing a minimally invasive nullconcentrating in the neck area along with doing some work under the chin to release the bands. This approach is more invasive and another way of improving the way your neck looks is by grafting fat throughout your neck. You have to have enough fat to graft into the area. Usually the abdomen or stomach area is the best area to get this fat.

microdermabrasion for wrinkles?

There are many types of microdermabrasion machines out there and they vary in terms of how much pressure is created. You can get a superficial peel from the most aggressive types and this can help with fine wrinkles. Again, mulitple treatments may be required to improve there appearance. Fractional resurfacing can help the vertical wrinkles. Volumizing can also help.
