What can fraxel, ipl, chemical peels do to help Acne?

What can fraxel, ipl, chemical peels do to help Acne?  The patient who asked this question also mentioned that another doctor told him that the scars eventually all reform and recreate the past scars.  What can be done for this patient.

Acne scars (Dr Young Bellevue Washington) can be improved through excisions, resurfacing, fat injections.  It all depends on the type of scar you want to improve.  If there involves a significant difference in the level of the scar you need to improve that mostly by excision.  This is more so with depressed scars.  There are many ways to try to elevate the scars including very strong chemical peels in the depressed scars which will elevate the scars. Subcision is a technique that can be used to incise around the depressed scar releasing the tethering component to allow the scar to rise up to the level of the surrounding skin.  Or just excision which is the better choice because you make the ultimate incision smaller.  Resurfacing after the subcision or excision comes next and this is where the incisions are made to look less noticeable.  Sometimes through acne, scarring is extensive in the subdermal and subcutaneous plane causing large areas to be adhesed to the deeper tissue.  Filler can separate this scarring but the best thing for this is fat injections.  Injecting fat into this large layer of scar will soften the adhesions and allow the skin to become more free.  The extra volume also allows the skin to be taken up more reducing more lines and wrinkles and a coalescence of scars to make larger scars.  Also with the layer of fat, the skin can begin to heal and remodel itself to make the skin look better.

IPL can decrease any vascularity from the acne. Fraxel can shrink some parts of the scar. While chemical peels can remove some of the scars that are more superficial.  But my preference is to excise and then resurface with a co2 laser.  I also consider fat grafting underneath the skin to allow it to heal. I personally don’t think fraxel works well for Acne scars.  IPL can help with the vascularity of the scars if they are new or possibly any pigmentation issues of the scars but that is the most it will do.  Chemical peels can help like any resurfacing but can be limited without changing the depressed or raised nature of the scars.

Heidi Montag’s recent 10 procedures in one day topic

Its big news that Heidi Montag had 10 procedures in one day.  In my opinion, that might be a little too much surgery in one day.  Although all of this information may be false. Who really knows but her and her doctor and the people in the room that day how many surgeries she really had.  I think based on looking at her pictures that she did appear to have had chin reduction, a rhinoplasty, possibly a browlift, and cheek enhancement.  Based on the pictures, I think she looks better before all of her procedures.  The chin reduction surgery has made her face actually look more square and more masculine. Although she mentions that she was teased as a child about her chin, I think the chin gave her lower face a pointier appearance which as an overall affect made her face appear more pear shaped.  Now that the chin is reduced, the face has lost the pear appearance and has been squared off like many male faces.  Her changes in her nose, possibly due to a rhinoplasty, has actually lengthed the appearance of the middle part of her face.  With that change, her face looks longer and less proportionate.  the distance between her eyes, nasal tip and lower lip should all be separated by equal distances and ideally 3 iris widths.  Her nose looks longer and I think her face has loss some beauty points. I think she might have also has her cheeks augmented.  This appears to have made her cheeks look wider.  I think that has also masculinized her face.  The cheek augmentation should have been more anteriorly closer to the eye and just outside (or lateral to) the eye.  The browlift, if it was done, is questionably needed at best.

When I see patients that request things that I don’t think needs to be done, I will tell them exactly what I think.  If I feel strongly enough, I refuse service to them.  If the procedure they are requesting is not consistent with my beliefs of what would enhance their aesthetics, it takes a lot of influencing for me to do what they request.  Although, I always have to balance my aesthetic beliefs with the desire of the patients.  Ultimately, what the patients wants is still very important.  But I have a long discussion with them if their desires are quite different then what I think will help them.

Nasolabial Fold Grafts are another option to improve the Nasolabial folds by Dr. Philip Young of Bellevue | Seattle: Nasolabial folds are a very common structure that develops as one ages and it is one of the most common structures that people want to get rid of.  I’ve learned a lot of things about improving this structure.  Facelifts have been experimented with over the years and most people believe that the deep plane facelift when the SMAS is lifted over the smiling muscles is the best way to improve the nasolabial folds from a facelift prospective.  Others find that a midface lift does this better.  Others find that volumizing the cheek in certain ways improves the fold as well.  Fillers have been used in the nasolabial folds to improve them and this is a very common option that people have chosen. My opinion is dependent on the person’s anatomy.  If the person has a lot of tissue and has some extra weight that they have gained.  Volumizing might make the face a little too big.  In this case a facelift can help improve the nasolabial folds.  One thing that can help the folds is too fill the volume up with a nasolabial fold graft.  This graft can be taken from the SMAS (superficial muscular aponeurosis system) which is the layer of tissue that is connected to all of the facial muscles. The SMAS is convenient because this the layer that the facelift uses to pull the face and can be easily taken to use for the nasolabial fold graft.  This graft can be placed in the nasolabial folds through the nose and can help improve their appearance along with the deep plane facelift.

If a person is against such a surgical procedure than I think volumizing might be the best choice for them.  This can be done with essentially no incisions.  The YoungVolumizer is great for this.  If undergoing a procedure like this is too involved for the particular person, then volumizing with fillers such as restylane, radiesse, juvederm, and perlane are options.  Sculptra is another long term permanent filler option that can also be used to improve and volumize the face to improve the nasolabial folds.  In terms of fillers and volumizing, there is a special location, an area that contains a deep fat pocket that really helps to decrease the nasolabial folds when it is volumized.  It takes a lot of experience to learn how to fill up this fat pocket with fillers and other volumizers, like the “The YoungVitalizer” procedure.

Reducing forehead wrinkles

The aging that occurs in the forehead entails a volume loss and it also entails some loosening of tissue and ligaments that hold structures in place.  As the eyebrow descends, we tend to elevate them to improve our vision and this can lead to wrinkling in the forehead area. This situation is coupled by our loss of volume (fat, collagen, bone loss).  This loss of volume places the vicinity of our skin closer to our muscles in our face.  Hence when we move our muscles in our face, like our forehead muscles, the muscles exert more action on our skin just by the fact that they are closer to the skin.   Through volume we can create a distance between the muscles and our skin and hence decrease the influence of the muscle on creating wrinkles. The question is then does the volume make us look weird.  The answer is simply “no”. When we were young, we had more volume all throughout our face.  If you take a look at pictures of yourself when you were young, you will notice a lot of loss of volume in your face.  Take a look at your temples, forehead, and your cheeks.  It will take a little time to adjust your thinking but you will see the difference. This loss of volume places your muscles closer to your skin. This is one of the main reason for forming wrinkles.  The amazing thing is that through volume, you can actually affect your skin change it for the better.  With the volume, the skin is allowed to remodel itself.  Your skin is in fact always turning over the cells and tissue within its structure.  With the decreased influence of the facial muscles on the skin, the cells within your skin begin to remodel and decrease the wrinkles that it has created all of those years.  This process of skin rejuvenation after volumizing can take many years and the change will be gradual.  This is a couple of reasons why the YoungVitalizer is my procedure of choice for facial rejuvenation.

In terms of the forehead, the volume can elevate your eyebrows to a state that you were a long time ago.  The volume again will also improve your wrinkles as well.  Volumizing is the most natural way to rejuvenate your forehead.  When I do forehead rejuvenation and rejuvenation of the upper eyes, I place the volume deep below the eyebrows, inferior to the eyebrow and within the upper eyelid, above the eyebrows and also in the temple and forehead areas.  This volume helps lift the eyebrows and can brighten the appearance of the eyes better than any other form of facial rejuvenation.

Browlifting is another way to do so.  You can elevate your brows and this can decrease your need to lift your eyebrows and ultimately your wrinkles can improve.  Also through browlifting some of your muscles can be altered to weaken them.  This is useful in the area in between your eyebrows and eyes.  Browlifting however can lead to less than a natural appearance.  Your eyebrows can be elevated to a point that is higher than they use to be. Also the removal of the muscles in between your eyes, although it will help decrease the movement there, the volume loss can make this area look worse.  If you want your eyebrows higher than they used to be, a browlift might be the better option.  But this is probably the only option that is better than volumizing in my opinion.

The YoungLift is being changed to the YoungVolumizer

This blog discusses our name change from the YoungLift to “The YoungVolumizer”.  We had to change the name for various reasons.  My wife actually came up with the name YoungVolumizer.  I was thinking about the name for a while and when she told me her idea of the YoungVolumizer I instantly liked it.  I then discussed it with the rest of my staff and they felt that it was also a great name.  We are making people look younger by volumizing their face. We hope our readers and followers like this new name.

What is a good option for something in place of Sculptra? What are the alternatives?

Fat injections are the best option as an alternative to Sculptra injections. I even consider fat injections first before Facial Fillers. Nothing is better than using your own tissues for plastic surgery purposes.  You have essentially no risk for reactions, no risk for cancer, no toxicity from your own tissues. Sculptra is made up of suture material. It is a foreign substance that is essentially broken down into carbon dioxide and glucose and ultimately carbon dioxide and water.  Other ways of filling is through silicone implants. The problem areas for implants and Sculptra is the periorbital area.  Implants are also hard to use for the buccal area where there is no bone for the implant to rest on.  It is here that Sculptra can be used and Fat injections.

Can you Fat Graft / Transfer Around a Facial Implant? Can you Fat Graft / Transfer / Inject into the area if you remove the Implant?

You can graft around an implant but it depends on the blood supply around the implant.  Typically, around the implant a layer of scar tissue usually forms. Scar tissue is notoriously known for its low blood supply.  Bringing fat next to the implant has the potential to live if done right.  Over time with the survival of fat around the implant, additional fat grafting (or Fat transfer / Fat Injections / Fat Transplants / Fat augmentation / Fat enhancements) can be done there to further increase the volume around the implant. If you remove the implant, the success of fat grafting can be attained but within the scar tissue, the fat has a more difficult time to survive.  This may mean that you will need to do more touch ups to the area.  Fat grafting can lead to long term results.  It is all about the technique.  In my practice, fat does survive and sometimes near 90% of the fat can survive.  This is true, so much so, that I need to be careful not to put too much fat in to certain areas.  It is easier to put more fat into the area than to take it out.     Fat injections (fat filler, fat transfer, fat grafting, pearl fat grafting) done right will not make you look fat just younger. Repeating the lower eyelid blepharoplasty and doing fat grafting is not a familiar course for me. I either do one or the other and I tend to do more fat grafting.  I only consider the lower blepharoplasty with fat grafting when there is an extraordinary amount of fat in the lower eyelids and this is not common.  Most of the time when you remove fat and skin from the lower eyelids you tend to look hollow there.  Fat grafting as a whole can really help this area by bringing back the volume to the lower eyelid and upper cheek area.  Also combining the two procedures can have an impact on the fat survival in my experience.  With fat grafting, I always have patients bring in photos of themselves when they were between 10-20 years old.  This helps me to determine where to put the fat.  Fat grafting will not necessarily make you look fat at all. When placed correctly, it can dramatically make you look younger.You just need to put the fat in the right places. Also browlifting is more complicated than most surgeons think.  A little can do a lot to make someone look surprised.  Many times fat grafting around the eyes, temple, and forehead can really make someone look younger in this area while elevating the eyebrows or giving the illusion that it has been elevated.     What percentage of body fat do you need to be a candidate for fat grafting?

    Studies looking into the percentage of body fat are not prevalent in the literature. So this hasn’t been determined accurately. Because you work out regularly and have low body fat, you might not be the best candidate for a lot of Fat Transfer(fat injections, fat transfer, fat filler, fat injectible). I would reserve fat grafting for areas that are least likely to tolerate more permanent fillers or implants.  These areas would be around the eyes and in the cheek hollow area.  The alternative for the areas other than that are sculptra, silicone implants for a more semi permanent or permanent options.  Temporary fillers can be used everywhere essentially.  Temple hollows, forehead volume loss, jaw shrinkage and sunken cheeks can be augmented with silicone implants as an option in the very thin or fit patient.  Fat can then be used in the areas that cannot tolerate these implants.

    Fat Grafting to Rejuvenate Aging Hands

    Fat grafting into the hands is a great way to rejuvenate the way your hands look in the aging hand.  When you age you tend to lose the thickness of the skin and also the fat tissue within your hands. This eventually exposes the veins and the tendons in your hands.  Many people feel, surgeons and lay people, that the face and the hands are the easiest way to figure out how old a person is.  The options for rejuvenating the hands include fat injections, temporary fillers or other grafts including alloderm, whole fat grafts, special skin grafts, radiesse, restylane, sculptra, etc.  Using implants that are not part of the body can cause problems and sometimes can have an unnatural feel.  Fat grafting is a great option to rejuvenate the aging hands while using your own tissue.  When you come in we try to determine what you are looking for when it comes to rejuvenating your hands.  We then discuss some pictures of hands to get an idea where you would like to be.  Then we discuss where we will place the fat in your hands.  At this point we can determine if we want the fat just between your tendons or also around your knuckle area to disguise these better.  We also discuss where we will take the fat from.  We harvest fat from almost anywhere in your body, including the abdominal area, the hips, butt, flanks, back, inner and outer legs and arms depending on your desires.  This is the same process that we go through when we discuss fat grafting in your facial area.  We also discuss what kind of anesthesia you would like including 1. just local anesthesia, 2. oral sedation with local anethesia, 3. iv sedation with our registered nurse with local anesthesia, and 4. more heavy iv sedation with our certified registered nurse anesthetist.    The process of fat harvesting and implantation I use is termed “Serial Timed Fat Harvesting and Transfer”.  This is a technique that I developed to maximize fat survival incorporating the latest techniques that are found across the world.  I harvest the fat with small cannulas through a very small puncture/incision under low pressure.  Then the fat is then spun in a centrifuge at the lowest speed and for 3-5 minutes.  This separates the fat into 3 layers.  The fat is then refined removing the top and bottom layers which are lysed fat cells in the top and liquid and anesthesia in the bottom layer.  The middle layer which is the most important layer of fat cells is preserved and used for fat transferring.    We then inject the fat through tiny puncture holes that heal like mosquito bites and this is done in a careful layering process to insure survival (Figure 1).  Our “Serial Timed Fat Harvesting and Transfer” is our secret which we have found has markedly increased the survival of our fat and we employ this with our fat grafting to the hands and the YoungLift for the Face.  For each hand we typically harvest 60-80 cc or milliliters and then inject approximately 10-30cc into each hand with the average being around 20 cc.
