Lower Facelift or fat grafting and questions on the incision in front of the ear

Saturday, June 27th, 2009

The incision in front of the sideburn can avoid moving the sideburn back. This would be the only way to avoid moving the sideburn up.  So the decision is up to you what is more important, moving the sideburn up or a scar in front of your sideburn. Now for males, their beard hair can be adjusted with shaving to grow a new sideburn. But for females, when you move the sideburn up you will not have the sideburns back to completely normal after the facelift. Fat injections under the eyes and cheek area is a great way to make yourself look younger in a natural way. I would really like to look at a photograph to be able to tell you.

I refined an amazing volumizing procedure called “the YoungLift”.  This is an innovative technique that volumizes your face to bring out the younger and natural you without looking like you had something done.  This is not a facelift and requires no incisions.  Only pinpoint puncture sites are used and these heal imperceptibly.  The YoungLift can be done without general anesthesia and without drains, and large bandages.  You also have a lot less downtime and discomfort compared with traditional facelift procedures.  Dr Young employs the very best techniques from around the world into one volumizing procedure and he uses his internationally acclaimed understanding of facial beauty to create the youthful volume you once had.

Fat injections into the nasal and nose area

Saturday, June 27th, 2009

Fat can be injected in the nose. Because the nose is at the center of your face the margin of error is very small. You should definitely consider going to someone who has a lot of experience doing this type of procedure.

Fat Transfer have an immense variability in terms of the results that you can get from it. There are many steps that go into fat injections that can insure that you have reliable fat survival. This reliability is essential to knowing whether the fat will survive in the areas that you implant it. If some survive and some fat cells don’t, the possibility of having variable results is possible. But there are always remedies. It is better to be on the conservative side so that you don’t inject too much. It is always easier to put more fat in than to take too much fat out.

One thing that is important to realize is that injections around the nose can be more dangerous than other areas. There are larger veins in this area, and also, the veins here don’t have valves and can serve to allow fat to travel backwards into deeper structures. This can lead to significant complications. You have to be aware of the literature to know how to prevent these things from happening but there is always the chance of things happening with anything that you do.

Fat grafting into the lower eyelid as a treatment option

Saturday, June 27th, 2009

In my opinion, fat transfer in the lower lid can do wonders. they have to be done right though.  If not done right you can get lumpiness and noticeable irregularities.  These issues can be addressed though and are not a significantly bad thing to have and take care of.

Fat grafting in the lower eyelid and cheek area is the best way to rejuvenate this area out of all options in my opinion.  The aged appearance in this area is mainly due to a loss of volume and replacing that volume can lead to the most natural and younger appearance.

Lower blepharoplasty with removal of skin and fat makes people look hollow and unnatural but I won’t argue that they still look a little better.  I just beleive that people can look a lot better by volumizing this area.

Dr Young employs the very best techniques from around the world into one volumizing procedure and he uses his internationally acclaimed understanding of facial beauty to create the youthful volume you once had.

dark greenish discoloration of the skin after fat transfer

Saturday, June 27th, 2009

These could be iron deposits after bruising where the iron is left behind. Sometimes, mild heat and massage can help this as well as exercise or increasing the lymphatic drainage in this area.

Sometimes, it can be due to prominent vessels that are residual after the procedure and the stimulation caused by the procedure post operatively. Or it could be that your Fat Transfer just elevated this area so that you can see it better.

What could be causing this is that your eye muscle is showing through your skin more given the thin skin in this area. The treatment is increasing the distance and adding layers between your eye muscles and the skin through fillers and more fat injections superficially between the skin and muscle. Also, CO2 resufacing can create an extra layer of collagen that can disguise the transparency of the skin that allows the deep color of the muscle to show through.

Will massage make the fat go away?

Saturday, June 27th, 2009

This was a question someone had regarding a Fat Transfer procedure they had done in the lips and whether massage would make the fat go away or lessen in the lips because she felt that there was too much put in the lips.

Massage is not likely to make a huge difference and at best it will make the swelling go down faster. Your upper lip should never be bigger let alone greater than 1/2 the size of your lower lip. My theory on aesthetics or facial beauty, Circles of Prominence, has discovered that your upper lip should be about 1/2 the size of your lower lip. Your swelling may go down quite a bit. Hopefully most of that will go away.

Give it some time and try not to worry too much about it. You can make the fat go away through microliposuction with the same cannulas that you put them in with. Sometimes you can also excise the extra fat out and different types of excisions can be done to make the lip look better and smaller. The swelling will likely go down quite a bit though.  It can be done though!

Do fat injections dissolve when you lose weight?

Saturday, June 27th, 2009

Fat Transfer are a great way to volumize the face. The survival of the fat is largely dependent on the technique that is employed. Once the fat finds its own blood supply it will survive as it did where it originally resided. Research has shown that we are born with the same number of fat cells in our body and the fat cells that we have get larger and smaller based on what our weight is. When the fat is placed into the face, typically it is believed that the cells are donor dominant. This means that the fat is likely to follow the behavior that it had where it came from originally.

When you lose weight, you will lose it everywhere. There are some areas that you will lose more than others depending on the activity. But, in general, you are likey to lose it everywhere in a symmetric manner. The same principle applies to the grafted fat in the face. But, because you have transplanted more fat cells to your face you will always have more fat in the area that you have transplanted more fat cells.

Based on research that has been done, the greater number of fat cells in your face will still give you more volume in those grafted areas. But, you will have less volume with weight less proportionate to the amount of weight that you lose.

Fat injections to the cheeks and persistent hollowing near the ears?

Saturday, June 27th, 2009

This patient had a Fat Transfer procedure somewhere else and was concerned that she didn’t have enough fat near her ears.  She had fat placed in her cheeks and it made the lines around her mouth better though.

Sometimes, augmentation in one area may make another area appear under augmentated. The swelling should go down and reveal the final results in 6 months to a year. Your doctor will be able to tell if the volume is due to one area being larger and more swollen then the other and whether adding additional fat would be necessary.

You can always add more fat. Taking out too much fat is a little harder than adding more fat at a later date. The best way for me to tell is to see pictures and you in person.  The way you put fat in and where can mean a world of difference.

Fat placed in the wrong area and how to treat this?

Saturday, June 27th, 2009

This was a question that I answered for a patient who had a Fat Transfer procedure by another doctor.

You might need a touch up to place more fat in the area that is depressed. Or you can remove the fat that is above and below which may be more difficult. You should discuss this with your surgeon. He will hold your hand so to speak through this and help you get the best results.

Touch ups are usually less involved and will take less time.  Sometimes the more elevated areas above and below could be due to swelling that will come down.  Patience is required in this case and it may get better with time. Your surgeon will be the best to counsel you on this. Don’t worry, it can be improved.

Mid face lift or other options for a tired looking face, nasolabial folds and under eye hollowing

Saturday, June 27th, 2009

Fat transfer could be your answer. Many times, the cheek area and lower eyelids appear the way that they do secondary to a loss of volume in those areas. Fat transfer is the best thing to make the area look natural. It’s not magic, but when done right and with enough attention, it can seem like magic.

A midface lift, in my opinion, results in less than optimal improvement of the area. You can’t really take that much skin from a midface lift. Also, with a midface lift you can end up with eyelids that are pulled down like a hound dog. It also doesn’t look natural and you often get bunching in the lateral eyelid area. I think that a midface lift is only beneficial when you need to improve the lower eyelid when there is a problem with the lower eyelid function. Once you plump up your midface with fat, you can concentrate on improving the acne scars with different types of procedures including resurfacing and excision of your scars.

Facial Implants can help restore loss volume. They require small incisions in the mouth or other ares but mostly for the midface the implants can be placed through the mouth and sometimes through the lower eyelid area.  Silicone implants through Implantech are the types that I usually place.  These implants have a long history of success with low rejection and reactions.

Is there a non surgical facelift that works?

Saturday, June 27th, 2009

There are different options to rejuvenating a person’s face. You don’t necessarily have to undergo a major null with long incisions to make yourself look better.

A great option is facialFat Transfer.  When people age, they lose volume in the face and this contributes to the facial skin drooping and the wrinkles to form.  It has been the practice of surgeons to cut away skin that appears to be excessive to what it used to be.  This approach is not always the best way to make a person look younger. Sometimes the extra skin is the result of the face losing volume and the skin sags and droops due to this process.  It is analogous to a grape losing volume when it turns into a raisin.

Traditionally, surgeons have practiced reductive procedures, meaning they reduced what is left. This can make a person look better but oftentimes the person doesn’t necessarily look younger.  A person will look like a more shapely raisin instead of a younger and healthy looking grape.

Fat injections can restore this volume and help you look younger in a natural way and it can be done with no incisions.  Most all of the time, all that is needed are very tiny puncture holes for the special instruments used to restore volume into the face.  Other ways to bring youth to your face without surgery include a resurfacing procedure to remove wrinkles, fillers to fill in volume in a piecemeal way over time, etc.
