One eye smaller after blepharoplasty

This is a question that I answered for someone who had a procedure by another physician.

There are many reasons why one eye would be smaller than the other. Swelling can be the most likely reason. No matter how well you do the surgery, you cannot control swelling sometimes. The body is a complex thing and many reasons could account for why one side is more swollen than the other after blepharoplasty.

Another probable cause is sometimes, manipulation of the muscle that opens the eye could have caused it to be temporarily tired. This could take months to resolve. Sometimes, if your surgeon has attempted to recreate your crease, there could by many more reasons for your eyes to look differently. This would depend on just what was done. You will have a tired eye just from the manipulation when you recreate the crease in double eyelid blepharoplasty which is done often in Asians.

Sometimes, when you create the crease too high on the muscle, it could tether it coming up and effect the movement of the muscle. You would really have to be seen by your physician to determine just what is going on. But wait it out for now and I wouldn’t worry about it until at least a month.

Fat injections complications resolved through a blepharoplasty

This particular person had lumps from Fat Transfer to the lower eyelid and upper cheek. This is a question that I answered for someone who had a procedure by another physician.

Lumps and Bumps can happen without almost anything that you do when you manipulate the tissues. When they are around the eyes, it’s pretty hard to mask their appearance. I can appreciate the concerns. There are many things that can be done to help this.

First, steroid injections can help by causing some of the fat to be dissolved by the body through the action of the steroids. There are risks with the use of steroids so you should be aware of these before undergoing these injections. This course may take several injections, and it may take a couple of months for the steroids to fully work.

The other option is to actually make an incision and take the fat out.

The third option is to actually carry out micro liposuction to take the fat away. Going to someone with a lot of experience with fat grafting can help avoid some of these issues, but even the best will have some lumps.

Fat grafting is a very complicated process for the surgeon and there a lot of steps that go into this procedure. Each step can affect the survival of the fat which can lead to lumps because some cells survive and some cells don’t. The more reliable the process you have the better chance you will get even survival of the fat and hence a better result. Its all in the technique!

Under eye loose skin and wrinkles after lower eyelid blepharoplasty

This is a question that I answered for someone who had a procedure by another physician.

The loose skin under your eyes can be improved by doing a pinch blepharoplasty technique without increasing your risk of your lower eyelids getting pulled down like a hound dog. This pinch blepharoplasty technique is very simple and can be done in under an hour and usually takes 30 minutes. I wouldn’t be too upset. I’m sure your doctor will be happy to do this for you.

One thing that can help those wrinkles and make you look more natural is too add volume under your lower eyes through dermal fillers or fat injections. This is a more natural way to improve the appearance of your lower eyelids and any sunken cheek type of appearance. Is a browlift or blepharoplasty a better procedure for someone

It depends on where your eyebrow sits above your eye. Your eyebrow should sit above your eyelid margin about the size of the width of your iris. So depending on where your eyebrow sits, you will be a better candidate for an eyebrow lift or just an eyelift.

If your eyebrow sits below the distance of the width of your iris, you could benefit from an eyebrow lift. If it sits above this distance, an eyebrow lift could make you look surprised. That distance determines whether you look suprised or maybe more stern-looking or tired-looking.

If you elevate the eyebrow much more than the width of the iris you could have that surprised look. A little more than that distance is okay, but once you start approaching 1 1/2 times the distance of the iris, the suprised look takes on more prominence. After elevating your eyebrows, you are then more able to determine how much skin to take.

Many times volumizing the upper eyebrow area is the best way to rejuvenate the upper eyelid area.  This can be the best option to subtly elevate the eyebrows and volumize the upper eye for the best and most natural results. Crepey Skin and Hollow Eyes after Blepharoplasty

This is a question that I answered for someone who had a procedure by another physician.

Lower eyelid blepharoplasty entails fat removal and skin removal from the lower eyelids. This is part of the reductive philosophy that has dominated surgeons, and plastic surgeons.

An analogy is that of a grape when it loses its volume and turns into a raisin. Based on traditional approaches, a plastic surgeon would take that raisin and remove the skin of the raisin to make it into really just a smaller grape. But based on this analogy, the smaller grape would not look like the grape that it used to. This same idea applies to a person’s face.

A person loses volume as they age. When people have their tissues taken in a reductive manner, they will no longer look the same as they did when they were young. The youthful face looks the way it does because of that volume. A person may look a little better after these reductive type of procedures such as lower eyelid blepharoplasty, but not necessarily better in a younger and natural way.

Volumizing a persons face and turning the face from a raisin back into a more youthful looking grape is the most natural way of making a person look younger in a natural way. What you can do for the sunken appearance is to bring back the volume. Fat injections is the most natural substance to use, and when applied to the lower eyelid and cheek, this procedure can make a person look a lot more natural and younger than lower blepharoplasty.

Headache after Neck Lift and Blepharoplasty

This is a question that I answered for someone who had a procedure by another physician.

Pains and aches after any procedure is a natural process and sometimes the cost of being beautiful. It is almost impossible to not have these after any procedure and this include a null and null. The question is how painful it is and if it is causing you to be in a state where you can’t function well on a daily basis. A stitch might need to be removed if it continues to cause you problems and can be done very easily.

The headaches are due to sutures pulling on your facial muscles and will get better. You might need some different medications to help you with your pain and transition.  Sometimes the pain is due to nerves being caught and is rarely indicated to have a procedure to alleviate this nerve impingment.  Muscle relaxants, anti inflammatories, Nerve relaxants can all help your situation and can be discussed with your doctor.  Don’t worry this will get better. Just be patient.  Your doctor will hold your hand, so to speak, through this whole process.

What do you do if you have asymmetric results after an upper blepharoplasty

This is a question that I answered for someone who had a procedure by another physician.

Asymmetry is common after double eyelid blepharoplasty but is avoidable by doing repeated measuring during the procedure.

  • When you go back to revise one eye it will look asymmetric for awhile during the healing phase and this is unavoidable and always the case.
  • You have to give it some time and not do anything until the healing has taken place.  If you do something during this healing phase it could make it look even more asymmetric in the long run.
  • I would wait at least 3-6 mos before considering any more treatment. Based on the pictures it looks appropriate right now and I think it will look great in a couple of weeks.

How to improve the appearance for the tear troughs

The question was asked of me whether null or restylane injections were better for the tear trough location. Both of those options can help. I think Restylane would be a better alternative to replace the volume that you have lost in the lower eyelid and cheek area. That is essentially what has happened — a loss of volume. The only thing is that restylane will last only 6-9 months. Fat repositioning can help the condition but there are limitations. It can leave you looking hollow and it can impact the position of your eyelids and can have complications.

I don’t prefer the fat repositioning procedure. I think volumizing the area is the best way to rejuvenate the face and the lower eyelid area. It can give you the most naturally looking results.  Fat transfer to this area is the best if done right.

Enlarged eye after upper and lower blepharoplasty

This is a question that regards a patient who had a bigger right eye after n upper and lower eyelid blepharoplasty and was wondering if it is due to the surgery and whether it will get better.  This is a question that I answered for someone who had a procedure by another physician. This persons right eye was larger than the left. This can happen with lower eyelid blepharoplasty when this procedure entails taking skin and fat. The skin resection and the scar contracture is what can cause the lower eyelid to retract downwards revealing more eye in that particular eye.  This can take a while to resolve on its own.   Sometimes, steroid injections can help.

In the long run, it may require other procedures to improve the retraction in the skin of the lower eyelid when the scarring occurs after lower eyelid blepharoplasty. This is one reason why I don’t prefer thenullas a procedure and believe in restoring volume to the lower eyelid and cheek region. I also prefer fat injections for this area for a longer lasting effect and more natural looking results. Sometimes this takes up to a year for the tightness to get better and for things to look better. Patience can be a virtue during this transition after a lower eyelid blepharoplasty.

How much is Arcus Marginalis Release and Fat grafting Cost?

Arcus Marginalis release is around the same price as null or a little more around $2k-5k.  It depends on what region the surgeon is and the surgeons reputation. There are many ways to price fat grafting some use amount per milliliter or by region but for the lower eyelid and cheek area is around 3-k for fat grafting in the lower eyelid area.

Arcus mariginalis release and fat repositioning for the lower eyelid is a way to improve the appearance of baggy eyelids and fat bags in the lower eyelid. This does improve the uneven levels in the lower eyelid but it doesn’t correct a fundamental thing–a loss of volume. Also, the complications are high with this procedure. You can have problems with your eyelid retracting, the fat often retracts back into the orbit, and the release of the volume of the orbit to the front of the orbital rim can make the upper eyelid more hollow by “stealing” volume from the orbit.  Fillers and fat injections into the lower eyelid region can improve this area in a natural way and is a great and better alternative in my opinion.
