What are the risks of blepharoplasty?

This is a question that someone asked me, here is my answer:

Whenever you have a procedure, such as a null(eyelift / eye lift / eye cosmetic surgery / eye plastic surgery / eyelid lift) , from a legal standpoint they will list everything under the sun.  The key is going to someone that has a low rate of complications.  But briefly, eye damage, damage to organs, nerves, vessels, poor results, scarring, eyelid malformations, infection, bleeding, anesthesia risks are among the risks that most surgeons should have in their consents.  99.9% of the time complications don’t happen.  You can read more on my blogs for more information and my website.

Asymmetric Eyes after Asian Eye Lift / Blepharoplasty

This is a question that I answered for a person with asymmetric eyes after Asian Plastic Surgery(Asian / Ethnic eye lift / belpharoplasty, Asian eyelid surgery, Asian Cosmetic Eye Surgery. She had multiple folds on the left side and the fold was shorter on the left as compared to the right.

I see that your left eye has multiple creases. I think what is going on is that the crease is not properly fixed on that side and needs to match the right side.  Sometimes these surgeries are difficult because you are so close to the eye when you approach the muscle that elevates the eyelid.  There is around 1-2mm of tissue thickness from the surface of that muscle and the eyeball.  That is not much room for any deviations.  Understanding the anatomy is vital in terms of identifying that muscle and fixing it to the skin edges to create the fold.  I think your situation can be corrected in a relatively uncomplicated manner.  I wouldn’t remove any skin in your situation and you might need to add skin and some fatty tissue to match the other side but I would start with correcting the crease.

When should flight attendants go back to work after an Eye lift?

This is a question a patient asked of me. Here is how I answered it:

One week is about the average time it takes to get better. Healing is personal and individualized after an eyelift / blepharoplasty (eye plastic surgery / eye cosmetic surgery / eye lift) .  Everybody heals differently.  Someone can heal to a certain stage at one week while someone may take a month to get to that same point.  Bruising typically resolves by one week and same with the swelling but everyone again is different.  In terms of bruising, additional bleeding the first week is the most important.  You should be really careful of not doing anything strenous for the first week but if you want to be aggressive to getting back to work the first two days are the most important for the bleeding and oozing.  One thing to realize is that your wounds heal 60% at 6 weeks, 80% at 6 months, and 88% approximately at 2 years.  This is another way at looking at things.  Based on this the decision to return to work is really up to you.  I have has patients take a flight 2 days after a facelift.  Everybody is different is the rule!

Blue shadows under the eyes and what are the treatments?

This is an answer that someone had asked me recently:How do you improve the blue hue under the eye with lower eyelid surgery and what options are there?

Options include fat injection (fat transfer / fat augmentation / fat grafting),  laser resurfacing, (laser peel / Active Fx, Deep Fx, Co2 laser resurfacing), lower eyelid blepharoplasty (lower eyelift / eye lift / lower eyelid pinch). It sounds to me that you might have some volume loss under the skin of the lower eyelid which would include skin thickness, collagen and fat in the lower eyelid cheek area.  The result is that the muscles around the eye are seen more clearly under the skin to give it the blue hue that is seen.  the ways to improve this include fat injections that create a layer of tissue under the skin to block the deeper blue colors from showing throw the skin.  Laser resurfacing also can create a layer of collagen underneath the skin to also block the blue from showing through.

The other explanation is that the bulging of fat from the lower eyelid could be causing a shadow under the bulge to cause the blue discoloration. Lower eyelid blepharoplasty which can remove the fat and skin that is causing the shadow underneath.

Eyelifts / Browlifts / Fillers and how to make the eye look younger again.

There are many procedures out there that can be done to improve the way the eye looks and to reverse some of the signs of aging (including droopy eyes, extra eyelid skin, tired looking eyes, puffy eyes, eye bags, eye wrinkles).  An eyelift / blepharoplasty (or eye lift / eye plastic surgery) procedure is one way and this entails removing skin from the upper eyelid. Sometimes this entails removing skin, muscle, fat and also recreating the eyelid crease.  Recreating the eyelid crease is perhaps the one thing that can really make an eye look better by reconstituting this crease which can really open up the eye and it appearance.  This is a really complicated part of an eye lift and requires understanding the anatomy to a greater degree and an increase level of expertise.  I have a lot of experience recreating the crease and I have done this with my Asian clientele but I also apply it to my caucasian clientele and other ethnicities.

Browlifts (or brow lift / forehead lift / brow plasty / browplasty / eye brow lift / eyebrow lift) are another way to open up the eye by pulling up the eyebrow to a more youthful position (other signs of aging include droopy eyebrows, hooded eye brows, extra eyelid skin).  This is done through incisions that are placed behind the hairline and hide really well.  Sometimes an incision in the eyelid crease is necessary to better expose the orbital bone above the eyelid and to allow a better approach to lift the eyebrow.  It is important for you to go to a surgeon that understandsFacial Beautyto get those excellent results.  You may have seen people who have a surprised look after a brow lift.  This is direct by product of our limited understanding of facial beuaty. What do you do for lower eyelid bags, dark circles for someone in their lates 30’s early 40’s?

Anullcan (or lower eyelid lift / lower eyelid blepharoplasty / lower eyelift / lower eyelid pinch) help by taking away extra skin and removing fat in the lower eyelid (this can also help lower eyelid bags, dark circles, puffy eye bags, dark under eyes). But instead of a lower blepharoplasty, I would consider fillers of some sort to the lower eyelid and cheek area.  What I see in many patients is the volume deficiency at the junction between your lower eyelid and cheek area.  When you age, you start to lose volume in the cheek area by losing fat and losing skin thickness and tissue within the skin.  A lower blepharoplasty could help the transition from your lower lid and cheek area. Uneven swelling and healing in the eyes after an eyelift / blepharoplasty.

Each eye can take different amounts of time to heal.  Blepharoplasty and eyelifts are essentially two different surgeries one on the right and one on the left.  Just like you are asymmetric on both sides.  Your anatomy is also not totally symmetric easier and that pertains to blood flow.  It is possible that your blood vessel anatomy is different from each side and that there may be more vessels stimulated on the side that is more swollen.  Also when sleeping you could have spent more time on the side that is swollen and fluid might have accumulated there more.  Don’t worry things will start evening out. I wouldn’t consider anything rash for a couple of months after eyelid lifting / eye lift / blepharoplasty surgery.

Best treatment for tear trough: lasers, fillers, or fat injections?

The tear trough is a tough area to treat through eye lifts / blepharoplasty procedures. This area is located between the nose and the lower eyelid and is situated not above the bone as most people believe but above the soft tissue of the eye socket and eyelid. People have tried repositioning of the eye socket fat into that area through a blepharoplasty / eyelift procedure.  This can work but has been shown to lead to eye position abnormalities and problems with dry eyes, etc. Fat Transfercan work in this area and may be the most natural way to rejuvenate this area.  It does have some issues related to survival of the fat.  But is the fat survives past a year it will likely stay in the area for many years, sometimes 10-15 years.   Fat grafting requires a lot of skill and knowledge of fat survival techniques.  Facial Fillers can fill up the volume loss that has lead to the tear trough appearance and I’ve done that many times and have a lot of patients that get this done.  The key is being able to fill up the area without leading to bumps and ridges.  This requires some skill and with a surgeon’s experience it can be done to look very nice and satisy your clients.  Fillers can look not as natural as fat, it can lead to a translucent type of appearance and some dark appearance of the filler within the skin.  But, in general >95% or more patients are very, very happy that they had filler placed to treat the tear trough area. Laser Resurfacing can create a layer of collagen underneath the eyelid skin to camouflage the dark appearance of the muscle underneath the skin.  It can also tighten the skin and reduce the unwanted wrinkles in the lower eyelid.   However, lasers will not replace the volume that is lossed with aging around the lower eyelid area.  This is where fillers and fat injections come into play.

Restylane or Arcus Marginalis release for lower eyelid dark circles

Both of those options can help. I think Restylane would be a better alternative to replace the volume that you have lost in the lower eyelid and cheek area. That is essentially what has happened — a loss of volume. The only thing is that restylane will last only 6-9 months. Fat repositioning can help the condition but there are limitations. It can leave you looking hollow and it can impact the position of your eyelids and can have complications.

I don’t prefer the fat repositioning procedure. I think volumizing the area is the best way to rejuvenate the face and the lower eyelid area. It can give you the most naturally looking results.  Fat transfer to this area is the best if done right.

Restylane for the dark circles under the eyes

Restylane will last about 6-9 months but can improve this area. Fat injections can last 8-10 or more years if the fat survives after the first year. You are on the right track though. You need volume in this area and that’s the best way to improve this area. One thing to realize is that restylane will go away and can also be taken out earlier if you wish through enzyme injections. This can be reassuring.

Fat injections are more permanent and is a little harder to get rid of if you don’t like the volume but can be done through different special procedures. In my opinion, fat injections / transfer leads to the most natural results.
